改良双环法巨乳缩小术后患者生活质量的随访研究

摘　　要：目的研究比较改良双环法乳房缩小术后巨乳症患者生活质量的改善情况，寻找影响生活质量改善的潜在因素。方法通过具有较强信度与效度的生活质量问卷调查表，分别调查患者手术前后生活质量方面8个维度的具体得分，得分越高表示生活质量越好。使用SAS9．1．3统计软件对手术前后所得数据进行统计分析，并对可能影响生活质量改善的潜在因素，进行相关性比较分析，尝试找出与生活质量某个维度改善率相关性最强的因素。结果术后患者生活质量较术前明显提高，8个维度中除“总体健康”外，其余7个维度得分均显著提高，具有统计学意义，其中以“情感职能”提高最为显著。肥胖与非肥胖患者的生活质量，手术前后均未发现显著差异。相关性研究分析并未发现与生活质量提高率有显著相关性的因素。结论患者术后总体综合满意度较高，是一种较理想的乳房缩小整形方法，同时表明，巨乳缩小手术并不仅仅是一项美容手术。Objective To compare the quality of life in women with symptomatic macromastia before and after reduction mammaplasty with the modified round block technique, and to find the possible factors which may impact the improvement of the quality of life. Methods The specific beneficial effects of reduction mammaplasty were evaluated with the modified round block technique, and a general questionnaire was used in this study： the Short Form-36 Health Survey Questionnaire （SF-36） which has good reliability and validity. The scores of the patients before and after the surgery were collected respectively. Statistical analysis and comparison were carried out with SAS 9. 1. 3 to find the correlations between the improvement rate and the possible factors. Results In all the patients enrolled, SF-36 showed significant higher quality of life after the operation with regard to 7of 8 investigated domains. Role of emotion increased most significantly. Increased self-esteem and increased personal and public self-consciousness after surgery were observed. We did not found any strong correlation between the improvement rate of the quality of life and any other factors. Conclusion Patients had a higher degree of satisfaction overall. These data are further evidence that breast hypertrophy is not solely an esthetic problem. The reduction mammplasty with the modified round block technique is an ideal technique.
