成人鼻咽腺样体肥大的影像诊断

摘　　要：目的探讨成人鼻咽部腺样体肥大影像学表现与诊断价值。方法回顾分析34例成人鼻咽腺样体肥大的影像学表现,所有病例均经活检和病理证实。结果鼻咽部侧位X线照片11例,表现为鼻咽腔顶后壁软组织增厚,范围11mm～17mm,绝对值均大于10mm。横断CT扫描23例,鼻咽后壁软组织厚度13mm-18mm,平均值14mm;所有病例均未见颅底骨质破坏。MR检查27例,鼻咽顶后壁粘膜增厚,厚度达5mm～10mm5例,11mm～15mm17例,15mm以上5例,均与头长肌分界清晰。5例可见1～4个咽旁或颈淋巴结增大,直径4mm～11mm。结论成人鼻咽部腺样体肥大的诊断不难;MRI具有良好的软组织分辨率,可分辨粘膜、腺样体增生肥大与肌层,诊断价值优于X线平片和CT检查。Objective To explore the performance and value of imaging diagnosis of the adult nasopharyngeal adenoidal hypertrophy. Methods Retrospectively analyzed the imaging performance of 34 cases the adult nasopharyngeal adenoidal hypertrophy, all had been proved by operative and pathological confirmation. Results 11 cases of pharynx nasalis lateral X-ray photographs showed the soft tissue of the posterior-top wall of the nasopharynx were thickening, 11 mm to 17mm, the absolute value was greater than 10 mm. 23 cases with transverse CT scans, the soft tissue of the posterior wall of the nasopharynx were thickening, 13 mm to 18mm, the average 14 mm. All cases were no cranial base bone destruction. 27 cases of MR examination, the mucosa of the posterior-top wall of the nasopharynx were thickening, 5mm to 10mm in 5 cases, 11 mm to 15mm in 17 cases, greater than 15 mm in 5 cases, and the longus scapitis boundaries to the mucosa was clear in all. 5 cases of that 1-4 enlargement of lymph nodes in the parapharyngeal or cervical, 4 mm to llmm in diameter. ConcIusion The imaging diagnosis of the adult nasopharyngeal adenoidal hypertrophy was not difficult. MRI has a good soft-tissue resolution, can discern the mucosa, the adenoid hypertrophy and the muscle.The diagnostic value is superior to X-ray and CT scan.
