IL-1β、幽门螺杆菌cagA与人慢性胃炎的关系

摘　　要：目的探讨IL-1β、幽门螺杆菌cagA基因(cytotoxin A)和人慢性胃炎之间的相关性。方法收集西安交通大学医学院第一附属医院胃镜检查的胃活检标本,经甲醛固定,制备石蜡切片,进行病理分型和IL-1βSABC免疫组化染色;同时作幽门螺杆菌培养,16srRNA PCR鉴定,检测cagA基因。结果慢性萎缩性胃炎组IL-1β表达的阳性率高于慢性浅表性胃炎组(P<0.05);慢性萎缩性胃炎组中,异型增生组IL-1β表达的阳性率高于肠腺化生组(P<0.05)。幽门螺杆菌cagA基因阳性率在慢性萎缩性胃炎组和慢性浅表性胃炎组无显著性差异(P>0.05),异型增生组cagA基因阳性率和肠腺化生组亦无显著性差异(P>0.05)。结论IL-1β在慢性胃炎的转归和发展中具有比cagA基因更高的预测价值。Objective To discuss the correlation between IL-1β, cagA of Helicobacter Pylori and chronic gastritis. Methods We collected the gastric biopsy specimens by gastroscopy for pathologic examination and culture of Itelicobacter pylori. In the meantime, IL-1β was detected by SABC histoimmunology and cagA gene was tested by polymerase chain reaction. Results The positive rate of IL-1β in chronic atrophic gastritis group was higher than that in chronic superficial gastritis group （P 〈 0.05）. In the chronic atrophic gastritis group, the positive rate of IL-1β in dysplasia group was higher than that in intestinal mesplasia group （P 〈 0.05）. There was no obvious difference in the positive rate of cagA gene between chronic atrophic gastritis group and chronic superficial gastritis （P 〈 0.05）. There was no obvious difference in the positive rate of IL-1β between dysplasia group and intestinal mesplasia group （P 〉 0.05）. Conclusion IL-1β has a higher value than cagA gene in predicting the prognosis of chronic gastritis.
