胰腺结核的CT诊断和鉴别诊断

摘　　要：目的探讨胰腺结核的CT表现,以期提高对该病的诊断正确率。方法经手术病理证实的胰腺结核患者7例,均行CT平扫及增强检查。结果CT检查发现胰头实质性或囊性占位5例,累及体部和尾部2例,类似胰腺癌,多为囊实性肿块,钙化少见。实质部分CT呈等或(和)略高密度,常轻度至中度增强,均匀或不均匀;囊性部分无强化,通常不伴有胰管的扩张,常伴有后腹膜或邻近淋巴结的肿大,且肿大淋巴结常呈环形强化。结论胰腺结核具有特殊的CT表现,环形强化的肿块不伴有胰管扩张,但伴有环形强化的肿大淋巴结为其特征,可以与胰腺癌等病变相鉴别。Objective To study the CT features of pancreatic tuberculoses and compared with the operative and pathological findings,in order to improve the knowledge and diagnosis. Methods Imaging CT features of 7 cases with pancreatic tuberculoses were retrospectively reviewed,and compared with operative and pathological findings. Results Pancreatic tuberculoses are rare diseases of pancreas,mostly affected adults with no sex predominance. The lesion mostly located in the head of the pancreas （n =5） ,also in the body or the tail with appearance as pancreatic carcinoma （n = 2）. Most lesions were solid-cyst tuner-like with no calcification. The solid parts of the lesions were slightly high density on CT with slightly or moderate enhancement. The cyst/necrosis part was not enhanced. There were no enlargements of the pancreatic duct. The lymph nodes near lesions were always enlarged with ring-like enhancement. Conclusion Pancreatic tuberculoses have typical CT features. Ring form enhanced lesions with ring-like enhanced lymph nodes without enlargement of pancreatic duet are characteristic CT features. And differential diagnosis from pancreatic carcinoma etc can be given.
