诺和灵N联合二甲双胍治疗2型糖尿病患者

摘　　要：目的了解中效胰岛素诺和灵N联合二甲双胍治疗对口服降糖药血糖控制不佳的2型糖尿病患者的疗效和安全性。方法对30例口服降糖药血糖控制不佳的2型糖尿病患者给予诺和灵N联合二甲双胍治疗,共12周。治疗前后测身高、体重、空腹血糖(fasting plasma glucose,FPG)、餐后2h血糖(postprandial glucose,PPG)以及糖化血红蛋白(HbAlc)水平。了解治疗期间低血糖发生情况。结果治疗后的FPG、PPG以及HbAlc水平明显下降,分别下降了3.36mmol/L、6.03mmol/L和1.75%,差异均有统计学意义(P<0.001)。治疗后40.0%的患者HbAlc水平<7.0%。治疗前HbAlc水平<9.0%的患者,治疗后62.5%的患者HbAlc水平<7.0%,治疗前HbAlc水平≥9.0%的患者,治疗后14.3%的患者HbAlc水平<7.0%,两者的HbAlc达标率差异有统计学意义(P<0.05)。治疗前后体重和体重指数差异无统计学意义(P>0.05)。30例患者共发生7次轻微低血糖。结论中效胰岛素联合二甲双胍治疗对口服降糖药治疗血糖控制不理想HbAlc水平<9.0%的2型糖尿病患者安全有效。Objective To investigate the effect and safety of the combination of mefformin with Novolin N in type 2 dia- betic subjects with poor glycaemic control by oral antidiabetic drugs. Methods 30 patients were treated with Novolin N and met- formin for 12 weeks. Height, weight, fasting plasma glucose（ FPG）, postprandial gincose（PPG） and HbAlc were measured be- fore and after the treatment. Hypoglycemia was also noted. Results After the treatment, the decrease of FPG was 3.36mmol/L, PPG was 6. 03mmol/L and HbMc was 1.75% , individually, which were obviously lower than the baseline （P〈0. 001 ）. At the twelfth week, the percentage of HbAlc〈7% was 40. 0%. The percentage of HbAlc〈7% was 62. 5% in the patients whose HbAlc was less than 9% at the baseline, however, the percentage was only 14. 3% in the subjects whose HbAlc was greater than 9% at the baseline, the difference was significant in the two groups（P〈0.05 ）, Before and after the treatment, no difference were noted in weight and BMI （P〉0. 05）. There were only seven phenomenon of hypoglycemia in the 30 subjects. Conelusion The combi- nation of metformin with Novolin N is a safe and effective therapeutic choice for the type 2 diabetic subjects with poor glycaemic control by oral antidiabetic drugs,
