儿童葡萄球菌烫伤样皮肤综合征临床分析(附149例报告)

摘　　要：目的探讨儿童葡萄球菌烫伤样皮肤综合征临床表现及治疗效果。方法对8年住院儿童葡萄球菌烫伤样皮肤综合征进行回顾性分析。并在治疗方面对于不同的治疗方法分两组,45例予以新青霉素Ⅱ联合其他青霉素,90例予以新青霉素Ⅱ联合头孢霉素治疗,比较其触痛消失时间、红斑水疱停止发展时间、红斑消退出现脱屑时间、治疗时间、病程等情况。结果新生儿并不多见仅有占4.7%。患病集中与1～3岁占43%,>5岁患病并不少见占16%。本组患儿中98例(65.8%)有明确的感染因素,其中培养出金黄色葡萄球菌15例,免疫功能检查52例,40例(76.9%)显示IgA低下;予以新青霉素Ⅱ联合其他青霉素或头孢霉素治疗,全部治愈,但联合头孢霉素治疗在临床疗效更好,两组比较差异有统计学意义(P<0.05)。结论葡萄球菌烫伤样皮肤综合征并不局限于新生儿和婴幼儿,较大儿童患病也较多。加强免疫、防止和治疗感染是重要的预防措施,首选有效的治疗药物仍是新青霉素Ⅱ联合头孢霉素治疗,在抗菌素治疗疗效不佳时可加用糖皮质激素。Objective Discussing the staphylococcal scald skin sydrome（SSSS） to draft the clinical manifestation and the treatment result. Cases in this group 98 cases （65.8 % ） have specific infection fac- tors, To cultivate one of 15 cases of Staphylococcus aureus. 52 cases of immune function tests, 40 cases （76.9%） showed low IgA; Be neomysin Ⅱ other penicillin or cephalosporin United adriamycin treatment, all cured.
