高渗氯化钠羟乙基淀粉40小容量复苏在低血容量休克及猝死患者抢救中的应用分析

摘　　要：目的:了解高渗氯化钠羟乙基淀粉40小容量复苏在低血容量休克及猝死患者CPR抢救中的临床效果。方法:将低血容量休克及猝死患者CPR分为常规治疗组(对照组)及高渗氯化钠羟乙基淀粉40治疗组,并观察治疗后30、60、90min血压达标(≥65mmHg)人数进行对比分析。结果:低血容量休克患者两组比较:30、60min有统计学差异(P<0.001),而90min及90min以后无统计学差异(P>0.05),两组综合比较有统计学差异(P<0.05)。猝死患者两组比较:30min时两组对比无统计学差异(P>0.05),在60、90min时两组分别统计对比有统计学差异(P60′<0.001,P90′<0.05)。结论:在低血容量休克及猝死患者临床抢救中应用高渗氯化钠羟乙基淀粉40小容量复苏方法简便,能在较短时间内恢复有效的循环量,缩短由于低血容量导致的组织器官血液灌注时间,阻断可能发生的后续多器官损害,为专科抢救效果奠定基础。Objective：To observe the therapeutic effect of Hypertonie Sodium Chloride Hydroxylethyl Starch 40 （HH40）on Hypovolemic shock and sudden death in CPR. Method：Divide the hypovolemic shock and sudden death patients found in conventional CPR respectively into control group（Conventional treatment group） and treatment group with HH40, then compare the number of two groups whose MA0P≥ 65 mm Hg after therapy at 30 min,60 min,and 90 min. Results ：The groups of hypovolemic shock ：there are difference at 30 rain and 60 min after therapy（P〈0. 001）, but difference disappeared 90 min and 90 min later （P〉0.05）;The groups of sudden death patients with CPR：there is no difference at 30 min （P〉 0.05）, but difference is found at 60 min, 90 min （P60′ 〈 0. 001, P90′〈 0.05）. Conclusion ： The HH40 method is simple and effective for the patients of hypovolemic shock and CPR for sudden death. It can help the pa tients resume the pathophysiologieal conditions of low circulating volume, shorten the period of hypoxia-isehemia in tissues and organs due to low circulating volume relatively,block the multiCorgan damage that may happened follow-up of, and lay the foundation for the rescue specialist.
