摘　　要：目的探讨颈椎前路减压植骨融合钢板内固定治疗下颈椎骨折脱位的疗效。方法对45例下颈椎骨折脱位患者进行前路减压、复位、植骨融合、锁定钢板内固定。结果全部患者获得随访,时间6～27个月,平均11.5个月。植骨于术后12周均获得牢固骨性融合,颈椎椎间高度和生理曲度维持良好,无术中、术后并发症,未见螺钉钢板松动、脱出、断裂现象,术后患者神经功能得到不同程度恢复。结论采用颈椎前路减压植骨融合治疗下颈椎骨折脱位,可使损伤节段获早期稳定,神经功能改善明显;前路颈椎钢板系统操作安全、简单、材料生物相容性好、内固定确定,为早期行颈椎前路手术创造了良好的条件。Objective To investigate the effect of anterior cervical decompression and fusion with bone autograft and internal fixation for lower cervical fracture and dislocation. Methods Forty-five patients with fracture and dislocation of lower cervical spine were treated by cervical anterior decompression, reduction,iliac crest autograft, fixed with cervical anterior locking plate. Results After surgery, the visit to the all cases lasted for 6 months to 27 months ,with the average of 11.5 months for each case. It was found that after the surgery,the patient＇s nervous function were improved ,the intervertebral height and physiology cures restored and maintained satisfactorily. No operation complications, internal fixation loosens＇, shedding or ruputure occurred. The implanted bone merged within 12 weeks. Conclusion The adoption of anterior decompression and bone autograft implantion in the treatment of lower cervical fracture and dislocation can keep the injured part stable ,neurological outcomes improved satisfactory ,anterior cervical vertebral screw-plate system is safety, simplicity,rigidity and can provide good conditions for early anterior cervical spine operation.
