急性冠脉综合征入院即刻应用辛伐他汀对炎症因子的影响

摘　　要：目的通过测定不同时间应用辛伐他汀对急性冠脉综合征(ACS)患者血清高敏C反应蛋白(hsCRP)、白介素-6(IL-6)水平的影响,探讨早期应用他汀类药物是否能更好抑制ACS的炎症反应。方法选取ACS患者60例,随机分为两组:A组在常规治疗的基础上于入院即刻口服辛伐他汀20 mg,以后20 mg/d;B组在常规治疗1周后加用辛伐他汀20 mg/d;用酶联免疫吸附测定法(ELISA)检测患者血清hsCRP、IL-6,用自动生化分析仪测定血脂。结果两组用辛伐他汀1周后,总胆固醇(TC)和低密度脂蛋白胆固醇(LDL-C)水平下降,但与用药前比无统计学差异(P>0.05),A组用药2周后TC和LDL-C水平下降明显(P<0.05)。A组用药1、2周后hsCRP、IL-6浓度较用药前下降(P<0.0 5)。B组用药1周后hsCRP、IL-6浓度较用药前下降(P<0.0 5)。A组在1、2周末hsCRP、IL-6浓度均低于B组(P<0.05)。血脂下降与hs-CRP、IL-6的水平变化无显著相关性。结论ACS患者早期应用辛伐他汀可降低血清hsCRP、IL-6水平,与降脂作用无关。辛伐他汀具有抗炎作用,早期应用他汀类治疗可使ACS患者获益。Objective To observe the influenee of imvastatin on serumin flammation cytokines IL-6、high-sensitive C-reaetive protein（hs-CRP） in Patients with acute coronary syndrome（ACS）,to know whether early intensive imvastatin treatment may yield more significant benefits in the patients of ACS.Methods Sixty Patients of ACS were randomly divided into two groups：GroupA（n=30） treated with imvastatin as soon as they come to the hospital,GroupB（n=30） treated with imvastatin from the second week,at the same time two groups treated with routine therapy from the first day to the end.The levels of Seruin IL-6、hs-CRP were measured on the first weekend,the second weekend by enzyme 1inked-immuno-sorbent assay（ELISA）.Results The concentrations of total cholesterol（TC） and low density lipoprotein-cholesterol（LDL-C） in two groups decreased 1 week after recieving imvastatin treatment（P〉0.05）、remarkably decreased in groupA 2 weeks after recieving imvastatin treatment（P〈0.05）,The concentrations of hs-CRP、IL-6 in two groups remarkably decreased 1 or 2 weeks after recieving imvastatin treatment（P〈0.05）,groupA had lower concentrations of hs-CRP、IL-6 than groupB（P〈0.05）.There was no correlation between the concentrations of hs-CRP、IL-6 and lipids.Conclusion Early treatment for ACS with imvastatin can remarkably decrease the concentration of serum inflammatory cytokines IL-6、hs-CRP,and negatively with lipids.early treatment with imvastatin may yield more significant benefits in the patients of ACS.
