超短波联合肺功能训练对慢性阻塞性肺疾病患者肺功能及生活质量的影响

摘　　要：目的探讨超短波联合肺功能训练对慢性阻塞性肺疾病（COPD）患者肺功能及生活质量的影响。方法将90例稳定期COPD患者随机分成联合治疗组、家庭训练组及对照组，每组30例。3组患者均给予常规内科治疗，联合治疗组同时辅以家庭肺功能训练及超短波治疗，家庭训练组则同时给予家庭肺功能训练。于治疗前及治疗6个月后检测各组患者肺功能及膈肌功能改善情况，同时采用圣·乔治呼吸问卷（SGRQ）对各组患者生活质量进行评定。结果3组患者经治疗6个月后，发现联合治疗组及家庭训练组FEV1占预计值％、FEV1／FVC％均较治疗前明显提高，SGRQ评分显著降低，并且以联合治疗组FEV1占预计值％及SGRQ评分的改善幅度较显著，与家庭训练组间差异具有统计学意义（均P〈0．05）；对照组FEV1占预计值％、FEV1／FVC％及SGRQ评分治疗前后均无明显改变；3组患者治疗前后膈肌功能均无明显变化。结论超短波联合家庭肺功能训练对治疗COPD具有协同功效，能显著改善患者肺功能及生活质量。Objective To investigate the effects of uhrashortwave therapy associated with home-based reha- bilitation on lung function and quality of life （QOL） in chronic obstructive pulmonary disease （COPD） patients. Methods Ninety patients with stable COPD were equally and randomly divided into a combination group （received uhrashortwave therapy combined with home-based rehabilitation and regular treatment） , a rehabilitation group （ received home-based rehabilitation and regular treatment） , and a control group （received regular treatment）. Spirometry and diaphragm function were measured, and St George＇s respiratory questionnaire （SGRQ） was administered for QOL assessment at the beginning and after 6-month of treatment. Results FEV1% pred and FEV1% increased and average SGRQ scores decreased in the combination group and rehabilitation group. FEV1% pred and SGRQ scores improved most in combination group （all P 〈 0.05 ）. FEV1% pred, FEV1% and SGRQ scores in control group did not show any obvious change. Diaphragm function in all groups did not change significantly. Conclusions Ultra- shortwave therapy combined with home-based rehabilitation might have combinative effects in improving lung function and QOL of COPD patients.
