Tei指数及Sa值在原发性高血压诊断中的异同点及相关性

摘　　要：目的:探讨组织多普勒Tei指数及Sa值在原发性高血压患者测量中的异同点、相关性及如何合理选择左室心肌测量位点。方法:高血压患者160例,其中左室心肌非肥厚组(Ⅰ组)及肥厚组(Ⅱ组)各80例,对照组60例。于二尖瓣环处取室间隔、侧壁、前壁、下壁4个取样点,测量各位点Tei指数、Sa值并取均值(Sm)。结果:(1)Tei指数在各组内不同位点间差异无统计学意义(P>0.05)。(2)3组组内位点间Sa值差异有统计学意义(P<0.01),测值排序依次为侧壁>下壁>前壁>室间隔。(3)对照组与Ⅰ组Tei指数与Sm无线性相关;Ⅱ组Sm和Tei值负相关(r=-0.47,P<0.05)。结论:(1)高血压患者二尖瓣环各位点Sa值不尽相同,尽量避免单点测量,如须单点测量应取前壁或下壁。(2)Tei指数可单点测量,准确且操作简便。Objective： To evaluate the difference and the correlation between the Tei index and the systolic mitral annular velocity （Sa） by tissue Dopple, and provide a more reasonable measurement site for left ventricle. Methods： One hundred and sixty essential hypertensive patients and 60 normal individuals were enrolled in this study. The patients were divided into two groups,group I （patients without left ventrieular hypertrophy） and group Ⅱ （patients with left ventricular hypertrophy）. The Doppler sample volume was placed at the septum, lateral wall, anterior wall and inferior wall of the mitral annulus. The Tei index, Sa value and the mean of Sa （Sm） at each site were measured. Results： （1） There was no significant difference between the Tei index at each site in the groups （P 〉 0.05 ）. （2） There was a significant difference on the sites of Sa value between groups （P 〈 0.01）,in which the order was lateral wall〉inferior wall〉mean〉anterior〉septum. （3） There was no correlation between the Tei index and the Sm value in control group and group Ⅱ. There was a positive correlation between the Tei index and the Sm value in group Ⅱ （r = -0.47, P 〈 0.05 ）.Conclusion： （1） The Sa values were different at four sites of the mitral annular. It is better to measure several sites and get mean of them. Anterior or inferior wall is a better choice if the single wall measurement is necessary. （2） The one wall measurement of Tei index could be used in normal individuals and hypertensive patients to make the measurement more convenient.
