经皮肾镜下3种能量方式碎石术的比较

摘　　要：目的比较3种能量方式的经质肾镜碎石术(PCNL)治疗肾结石的有效性和安全性。方法回顾性分析2006年1月至2008年3月在本院用气压弹道、气压弹道联合超声及钬激光3种能源方式进行了经皮肾镜碎石术的102例病历资料,比较3种方式碎石术的手术时间,结石清除率、术中术后并发症发生率。结果气压弹道联合超声碎石术组手术时间较气压弹道和钬激光碎石组明显缩短;气压弹道联合超声碎石术组结石清除率最高(92.8%),钬激光碎石组其次(74.4%),气压弹道组最低(51.4%),3个组两两间比较差异有统计学意义;3个组的术中和术后并发症率比较差异无统计学意义。结论超声气压碎石的PC-NL手术时间短,结石清除率高,有较高的有效性和安全性,钬激光碎石也较弹道碎石的PCNL提高了结石清除率。Objective To compare the efficacy and safety of 3 kinds of percutaneous nephrolithotripsy（PCNL） with different power mode. Methods The clinical data of 102 inpatients treated with PCNL during January 2006 and March 2008 were retrospectively analyzed,and the operative duration and stone-free rate and complications among 3 kinds of PCNL with pneumatic ballistic power and combined pneumatic and ultrasound power and holmium laser power separately were compared. Results The operative duration in combined pneumatic and ultrasound power group was significantly shortened compared with the other two groups. The stone-free rate was 92.8% in combined pneumatic and ultrasound group and 74.4% in holmium laser group and 51.4% in air-pressure group, and the differences among 3 groups were statistically significant. The complication rate both during operation and after operation were not statistically significant. Conclusion The PCNL with combined pneumatic and ultrasound power mode can reduce operation time and improve stone-free rate significantly. PCNL with holmium laser mode has the higher stone-free rate than PCNL with pneumatic ballistic mode.
