卡托普利与双氢克尿塞合用对血脂代谢的影响

摘　　要：目的:评价小剂量双氢克尿噻以及小剂量双氢克尿噻与卡托普利合用对高血压病患者血脂的影响。方法:针对基层高血压患者,投用小剂量双氢克尿噻(12.5mg/d)2周,根据血压达标情况加用卡托普利(99例,A组),或继续维持小剂量双氢克尿噻(67例,B组)治疗,随访3个月,观察用药前后血压、心率,血脂的变化。结果:两组的基线资料包括性别比、年龄、血压水平等无显著差异(P>0.05);两组治疗后血压、心率较治疗前均明显下降(P<0.05);经三个月治疗后,B组病人治疗后总胆固醇(TC)、甘油三酯(TG)、低密度脂蛋白-胆固醇(LDL-C)、高密度脂蛋白-胆固醇(HDL-C)无显著变化(P>0.05),A组用药后TC、HDL-C水平无显著改变,TG水平显著降低(P<0.05),LDL-C显著升高(P<0.05,但在正常范围)。两组治疗后TC、HDL-C等血脂水平下降幅度无明显差异(P>0.05)。结论:小剂量双氢克尿噻和小剂量双氢克尿噻加用卡托普利两种治疗方案对基层高血压患者血脂无显著影响。Objective： To evaluate the therapeutic influence of captopril combined low dose （12.5mg/d） dihydrochlorothiazide （DHCT） on blood fat in patients with essential hypertension （EH）. Methods： The 166 EH patients were randomly divided into two groups： Group B （67 cases） only received low dose DHCT, and Group A （67 cases） received low dose DHCT and captopril , treatment and follow-- up visit for 3 months. The change of blood pressure （BP）, heart rate （HR） and blood fat was observed. The distribution of baseline data （sex rate, age, blood pressure level） did not exist significant difference in two groups. Results： The BP and HR decreased significantly after antihy pertensive therapy in two groups （P（0. 05）. The changes of total cholesterol （TC）, triglyceride （TG） , low density lipoprotein-cholesterol （LDL-C）, high of density lipoprotein-cholesterol （HDL-C） were no significant in group B （P〉 0.05）. The levels of TC, HDL-C no significant change, the TG level significantly decreased （P〈0.05）, LDL-C level significantly increased （P〈0.01, but in normal range） in group A. There were no statistical differences in amplitude changes of all indexes of blood fat between two groups （P〉0.05）. Conclusion： The influence of low dose dihydrochlorothiazide or combine captopril on blood fat is no significant in patients with essential hypertension.
