摘  要:目的探讨外淋巴液部分丢失导致压力降低对耳蜗功能的影响,为相关临床手术和动物实验的安全性,可行性提供理论依据。方法人为外淋巴液瘘降低外淋巴压力但不损伤耳蜗内结构,记录60只豚鼠造瘘前及造瘘后2小时耳蜗复合动作电位(compound action potential,CAP)阈值和微音电位(cochlear microphonics,CM)幅度的变化。结果外淋巴压力降低可导致CM幅度轻度下降(P〈0.05),CAP阈值略有上升(约5 dB SPL)(P〈0.05)。结论外淋巴液部分丢失使外淋巴压力降低,在不损伤耳蜗内结构的情况下,可导致CM幅度下降。CAP阈值上升,但影响程度不大。在不损伤耳蜗内结构而仅丢失少量外淋巴液的条件下,相关的手术或动物实验是相对安全可行的。Objective To investigate the influence of the decrease in perilymphatic pressure on the cochlear functions of guinea pigs, and then to provide the theoretical framework for related clinical operation and animal experiments. Methods Cochlear fenestration was performed to reduce perilymphatic pressure without damage to the structure of Corti's organ. Then the amplitudes of cochlear microphonics (CM) and the thresholds of compound action potential (CAP) were recorded in 60 guinea pigs before and 2 hours after the operation. Results A slight decrease of the amplitude of CM(P〈0.05)and an increase of the threshold of CAP(P〈0.05)were observed after the operation. Conclusion A small amount of perilymphatic pressure reduction only has a slight influence on cochlear potentials; therefore, fenestration on the cochlea without damage to the structure of Corti's organ is a safe and feasible procedure.
