64层螺旋CT血管造影在腹部病变的应用

摘　　要：目的探讨64层螺旋CT在腹部肿瘤及血管性病变中的诊断及治疗价值。方法搜集Aquilion 64层螺旋CT进行腹部增强扫描,并经手术或病理证实的腹部病变患者24例,采用动脉期、门脉期及平衡期三期扫描方式,应用容积再现(VRT)、最大密度投影(MIP)、及多平面重建(MPR)技术进行动脉、门脉期血管重建。结果6例胡桃夹CT血管造影(CTA)表现为左肾静脉跨越腹主动脉(AO)处被肠系膜上动脉(SMA)压迫,SMA与AO的夹角变小;4例肾癌可见迂曲扩张的供血肾动脉分支;4例肾血管平滑肌脂肪瘤,CTA可显示肿瘤内的供血动脉及肿瘤引流静脉;8例与腹部动脉有关的动脉瘤,如夹层动脉瘤和假性动脉瘤、动脉瘤,CTA均显示清楚与动脉的关系;2例肠扭转,可见以SMA为轴心呈旋涡状旋转改变的典型征象。结论64层螺旋CT血管造影在腹部肿瘤及血管性病变中的诊断及治疗有很高的价值。Objective To evaluate clinical application of 64-slices spiral CT angiography In abdominal diseases. Methods Twenty-four patients with abdom inal diseases who were proved by pathology or operation, with Aquilion 64-slices spiral CT angiography The primitive data were reconstructed by volume reconstruction（VRT）,maximum intensity projection and multiplannar reconstruction. Results left renal vein（LRV） was compressed by superior mesenteric artery（SMA） and abdominal aorta（AO） ,angle of SMA and AO become small in 6 cases of nut cracker phenomenon. Renal artery was more wide and long in 4 cases of kidney cancer. Feeding artery and tumor vein can be well displayed clearly in Angiomyolipoma. Aneurysm such as dissecting aneurysm of aorta can be well displayed the relation with artery in 8 abdominal aneurysm. Typical phenomenon can be watched that gut rotate around SMA in 2 case of gut vovulus. Conclusion 64-slices spiral CT angiography can display well the vascular of tumors and the disease of vascular
