摘  要:目的应用斑点追踪成像技术评价慢性心力衰竭(CHF)患者心脏再同步化治疗(CRT)后左心室扭转及其与左心室收缩功能之间的关系。方法获取18例慢性心力衰竭患者左心室的标准短轴基底部、心尖部的图像,应用斑点追踪成像技术测量心底部旋转角度(Rot-B)及心尖部旋转角度(Rot-A),比较慢性心力衰竭患者CRT术后3个月与术前左心室心底部旋转角度差值(△Rot-B)、心尖部旋转角度差值(△Rot-A)、扭转差值(△LVtor)与左心室射血分数差值(△EF)之间的关系。结果CRT术后3个月左心室心尖部旋转角度较术前明显增加,左心室扭转角度明显增加。△LVtor、△Rot-A与△EF(r=0.619,r=0.592)呈正相关。结论左心室扭转为评价CRT后患者左心室收缩功能提供了有价值的信息。Objective To evaluate the relationship between left ventricular torsion and left ventricu- lar systolic function in patients with chronic heart failure (CHF)after cardiac resynchronization therapy (CRT) by speckle tracking imaging(ST/). Methods Two-dimensional echocardiographic images were a- chieved from the left ventricular short-axis views at the basal level and apical level of the left ventricle in 18 patients with CHF. The left ventricular basal peak rotation (Rot-B) and apical peak rotation (Rot-A) were measured at each level by STI. The difference of Rot-B, Rot-A left ventricular torsion (LVtor) within 3 months after CRT and baseline ( △ Rot-B, △ Rot-A, △ LVtor) were calculated. The correlation among △ Rot-B, △ Rot-A, △LVtor and the increase of ejection fraction(△ EF)were analyzed. Results The Rot-A and LVtor increased significantly within 3 months after CRT. The △ LVtor and △Rot-A were correlated posi- tively with △ EF(r = 0. 619, r = 0. 592). Conclusion The left ventricular systolic torsion is novel means for assessment of the ventricular systolic function in patients with CRT.
