蔗糖铁注射液和维铁缓释片治疗肾性贫血的对比研究

摘　　要：目的比较静脉应用蔗糖铁和口服维铁缓释片治疗维持性血液透析患者肾性贫血的疗效与安全性。方法选择运城市急救中心2007-05～2009-05慢性肾衰竭行血透患者30例,随机分为静脉组和口服组,各15例。静脉组采用每次血透时静滴蔗糖铁100mg,直至完成总补铁量,而后以每月100mg维持;口服组以维铁缓释片1片/d口服(每片含硫酸亚铁525mg),疗程均12周。观察并比较两组患者贫血治疗的效果、铁代谢指标的变化和不良反应发生情况。结果治疗后两组血红蛋白、红细胞压积、血清铁蛋白、血清转铁蛋白饱和度均有升高,静脉组上升幅度和速度高于口服组(P<0.05)。静脉组有1例患者(6.6%)出现不良反应;口服组有7例患者(46.6%)出现不良反应(P<0.05)。治疗前后两组肝功能、C反应蛋白等生化指标组内与组间比较差异均无统计学意义。结论静脉用蔗糖铁治疗血液透析患者肾性贫血的疗效和安全性明显优于口服维铁缓释片。Objective To compare the efficacy and safety of intravenous iron sucrose with oral vitferrum retard tablet in maintenance hemodialysis patients. Methods From May 2007 to May 2009,30 patients with chronic renal failure were randomized divided into iron sucrose group （group A, n = 15 ）and vitferrum retard tablet group （group B, n = 15 ）. The patients in group A were treated with intravenous iron sucrose during hemodialysis until total deficient iron was complemented. After that, the patients received 100 mg per month for maintenance. While the patients in group B accepted one vitferrum retard tablet orally per day. The course of therapy was 12 weeks. The results of anti-anemia therapy, iron metabolism index and adverse events were observed. Results All the baseline parameters were comparable between two groups. After treatment, hemoglobin, hematocrit, serum ferritin and transferrin saturation were significantly increased in both groups. However, the increase of these laboratory parameters was significantly higher in group A than in group B （ P 〈 0. 05 ）. There were less adverse events in group A than in group B （6.6% vs 46.6% , P 〈 0.05 ）. No difference was found in liver function and serum CRP between two groups before and after the treatment. Conclusion Intravenous iron sucrose is more effective and safer for treating anemic hemodialysis patients than oral vitferrum retard tablet.
