摘　　要：【目的】探讨凋亡抑制因子clusterin蛋白在肾透明细胞癌（RCCC）组织中的表达及临床意义。【方法】采用免疫组化（PV法）检测46例RCCC组织、10例正常肾组织及30例癌旁肾组织中clusterin蛋白表达，结合临床病理学及随访资料，分析它们之间的相关性。【结果】①46例RCCC组织中，clusterin无阴性表达，过度表达12例（26．1％）；10例正常肾组织及30例癌旁肾组织阴性表达4例，无过度表达。②不同临床分期的过度表达：T1、T2期7例（7／40），T3期5例（5／6），T3期的过度表达率明显高于T1、T2期（P=0．004）；N0期8例（8／40），N1和N2期4例（4／6），N1、N2期的过度表达率明显高于N0期（P=0．002）.③不同生存时间的过度表达：小于3年组5例（5／8），大于3年组7例（7／38），小于3年生存期组过度表达率明显高于大于3年生存期组（P〈0．01）。【结论】①clusterin蛋白过度表达与RCCC的临床分期呈正相关，提示clusterin基因与RCCC的恶性临床表型相关，可能作为判断RCCC恶性程度的指标之一。②clusterin蛋白过度表达的RCCC患者术后三年生存率降低，与病人的不良预后显著相关，clusterin可能成为RCCC患者的一个预后指标。[Objective] To explore the expression of clusterin in renal clear cell carcinoma（RCCC） and its clinical significance. [Methods] The expression of clusterin was detected by immunohistochemistry method. According to the clinicopathological data, the correlation between the overexpression of elusterin and pathologic data was analyzed. [Results]Clusterin had been found in all cases of RCCC. Clusterin protein .was present in the cytoplasm of both normal and cancer cells, but there was no nuclear staining identified in either type of cell. Overexpression of clusrterin was detected in about 26.1%carcinomas. The expression level of clusterin protein in RCCC tissues was significantly related to tumor clinical stage（ P〈0.05）. Recurrence-free survival in patients with strong clusterin expression was significantly lower than that in patients with weak clusterin expression. [Conclusion] The secreted form of clusterin may be involved in the progression of RCCC. The overexpression of clusterin is associated obviously with cancer＇s invasion and clinical phenotype and it may be used as one of the clinical molecular markers to determine the malignancy of RCCC. The prognosis of RCCC may be correlated with the overexpression of clusterin, and the protein may be of great value and significance in predicting the prognosis of RCCC.
