氯吡格雷时间依赖性抗血小板活性研究

摘　　要：【目的】探讨血小板二磷酸腺苷（ADP）受体拮抗剂氯吡格雷对经皮冠状动脉支架成形术患者，其抗血小板活性与时间的关系。【方法】接受经皮冠状动脉支架成形术患者30例，在阿司匹林治疗的基础上，先给氯吡格雷300mg负荷量，后给予75mg维持量，用光集合度计检测，不同浓度ADP和胶原诱导血小板凝集时，氯吡格雷抗血小板的活性。同时联合应用阿司匹林抗血小板。【结果】全部患者均在术后2～4h拔出鞘管，无穿刺部位显著血肿形成者。【结论】采用阿司匹林治疗基础上增加300mg氯吡格雷负荷量，随后75mg维持量的治疗，在摄入氯吡格雷后48h，其抗血小板活性达到峰值，且未见不良反应。[Objective] To explore the anti-platelet effect of clopidogrel, a platelet ADP receptor antagonist, for patients undergoing percutaneous transluminal coronary angioplasty and its relationship to the time. [Methods]Based on the aspirin treatment, 30 patients were given 300rag clopidogrel for load at first and then were given 75rag clopidogrel for maintenance. The anti-platelet effect of elopidogrel was detected by optical aggregometer. [Results] The extubate of all patients was completed at 2-4 h after operation.No hematoma at the puncture position occurred. [Conclusion] Aspirin combined with 300rag clopidogrel for load and 75 nag elopidogrel for maintenance can make antiplatelet activity reach the peak at 48h after the administration of clopidogrel.
