摘 要:目的探讨TNF-α和Fas/FasL系统的表达及骨髓细胞凋亡与再生障碍性贫血(AA)的关系。方法AA组30例分为重型AA组(SAA)和慢性型AA组(CAA),采用酶联免疫吸附法检测各组患儿骨髓单个核细胞培养上清液中TNF-α水平,采用流式细胞术测定骨髓CD34+细胞Fas、FasL表达率及细胞凋亡率。结果AA患儿骨髓单个核细胞培养上清液中TNF-α水平[SAA(27.9±8.20)ng/L;CAA(23.6±6.78)ng/L]显著高于对照组(t=3.432P<0.01);CD34+细胞Fas、FasL表达率分别为SAA(25.88±5.75)%,(5.87±0.56)%,CAA(23.51±3.13)%,(6.73±0.98)%,AA患儿CD34+细胞Fas表达率显著高于对照组(t=3.446P<0.01),但CD34+细胞FasL的表达率与对照组比较无显著性差异(t=1.083P>0.05);细胞凋亡率SAA为(24.6±9.56)%,CAA(20.9±7.32)%,显著高于对照组(t=3.492,3.458Pa<0.01);但SAA与CAA组间TNF-α水平,CD34+细胞Fas、FasL表达率,细胞凋亡率比较均无显著性差异(Pa>0.05)。AA患儿TNF-α与CD34+Fas+表达率呈正相关(r=0.542P<0.05)。骨髓CD34+细胞的凋亡率与CD34+Fas+表达率间也呈正相关(r=0.602P<0.05),但TNF-α表达水平与骨髓CD34+细胞的凋亡率间无明显相关性(r=0.086P>0.05)。结论TNF-α和Fas/FasL系统介导的凋亡参与了AA患儿骨髓造血干细胞的凋亡过程。Objective To investigate the relationship between apoptosis of bone marrow cells and tumor necrosis factor - α (TNF - α), Fas/FasL expression in children with aplastic anemia. Methods The concentration of TNF -α in supernatant of cultured bone marrow mononuclear cell(BMMC) was measured by enzyme linked immunosorbent assay. The expression of Fas/FasL, apoptosis ratio of bone marrow CD34^ + cell were evaluated by flow cytometry. Results The concentration of TNF - α in supernatant of cultured BMMC in AA group [ SAA ( 27.9 ± 8.20 ) ng/L; CAA (23.6 ± 6.78 ) ng,/L 1 increased significantly compared with control group ( t = 3. 432 P 〈 0.01 ). The expression of Fas/FasL in SAA and CAA group were (25.88 ± 5.75 ) % , (5.87 ± 0.56) % and (23.51 ± 3.13 ) % , (6.73 ± 0.98 ) %. Compared with control group, the expression of Fas was significantly different ( t = 3.446 P 〈 0. 01 ), but the expression of FasL was no different ( t = 1. 083 P 〉 0. 05 ). Apoptosis ratio of CD34^ + cell in SAA group [ ( 24.6 ± 9.56 ) % ] and CAA group [ (20.9 ± 7.32 ) % ] significantly increased compared with control group(t = 3. 492,3. 458 P 〈0. 01 ) ,but there were no difference between SAA and CAA group in terms of the concentration of TNF- α ,the expression of Fas/FasL and apoptosis ratio (Pa 〉 0. 05 ). The concentration of TNF-α was positively correlated with the expression of CD34^ + Fas ^+ in all types AA children(r = 0. 542 P 〈 0.05), and apoptosis ratio of bone marrow CD34^ + cell was positively correlated with the expression of CD34^ + Fas ^+ in all types AA children too ( r = 0. 602 P 〈 0. 05 ). However, there was no correlation between the concentration of TNF - α and apoptosis ratio of bone marrow CD34^+ cell( r = 0. 086 P 〉 0.05 ). Conclusions TNF - α and Fas/ FasL system can be involved in inducing CD34 ^+ cell apoptosis. This may contribute to understanding the decreased number of stem cells and bone marrow fail
