摘 要:目的探讨肺炎支原体(MP)肺炎患儿肺功能的变化及不同治疗措施对肺功能的影响。方法选择2007年3月-2008年7月在本院住院治疗的90例MP肺炎患儿作为观察组,同时选择年龄、性别与观察组相匹配的45例健康儿童为健康对照组。分别采用儿童肺功能仪测定2组儿童肺功能指标,并将90例MP肺炎患儿随机分为观察1组和观察2组(各45例),分别予不同治疗措施。观察1组单纯用阿奇霉素针剂10mg/(kg.d),加50g/L葡萄糖注射液100mL,1次/d,连用3~5d;症状控制后改为口服阿奇霉素10mg/(kg.d),1次/d,连用3d,停药4d,疗程3周。观察2组在观察1组的基础上加用孟鲁司特钠治疗3周,治疗前后作肺功能检测,并随访6个月。结果观察组患儿肺功能呈现呼吸道阻塞的改变,治疗后肺功能较治疗前明显改善;观察2组总有效率(97.78%)与观察1组(95.56%)比较,差异无统计学意义(P>0.05),观察2组病程[(7.27±1.44)d]较观察1组病程[(12.11±2.24)d]明显缩短(P<0.05)。观察组治疗前后肝肾功能比较无明显差异(P>0.05)。结论肺功能检测对小儿MP肺炎治疗效果的评价有着重要的作用;孟鲁司特钠佐治MP肺炎能缩短病程,减少病情反复次数,改善小呼吸道功能。Objective To explore the changes of lung function in children with mycoplasma pneumoniae pneumonia(MPP) and the responses to various treatments. Methods Ninety children with MPP from Mar. 2007 to Jul. 2008 were divided into observed 1 group and ob- served 2 group and in the mean time,45healthy children matched to the observed group in age and sex acted as the control group. The patients in observed 1 group were treated with Azithromyein injection 10 mg/( kg·d) ,once per day for 3 weeks;and the patients in observed 2 group were treated with Montelukast on the basis of the treatment in observed 1 group. Both observed group and control group were detected for the lung function with the child lung function meter and followed for 6 months. Results The patients with MPP showed small airway obstruction and the lung function improved after treatment. The effective rate of observed 2 group was 97.78%. and that was 95.56% in observed 1 group,and the difference was not significant between 2 groups ( P 〉 0, 05 ). The course of disease was significantly shortened in observed 2 group [ (7.27 ± 1.44 ) d ] compared with observed 1 group [ ( 12.11 ±2.24 ) d ] ( P 〈 0.05 ). The liver and renal function of all children after treatment were not of significant difference compared with prior treatment ( P 〉 0.05 ). Conclusions The lung function detection is important to the review of the effect of the patients with MPP; Montelukast, the leukotriene receptor antagonist, is very effective to shorten the course of disease ,to reduce the frequency of recurrence and to improve the small airway function.
