摘 要:目的监测2007年中国不同地区12所医院临床分离鲍曼不动杆菌的耐药性。方法收集临床分离的非重复鲍曼不动杆菌2718株,采用纸片扩散法测定菌株对抗菌药物的敏感性,采用WHONET 5.4软件分析数据。结果鲍曼不动杆菌对头孢哌酮-舒巴坦的敏感率最高,为72.1%,对亚胺培南和美罗培南的敏感率分别为60.6%和55.5%;对米诺环素和阿米卡星的敏感率分别为50.8%和42.9%。不同医院鲍曼不动杆菌对抗菌药物的敏感性有较大差别。全国12所医院药敏结果显示多重耐药鲍曼不动杆菌(MDRAB)已经达到47.7%(4.7%~64.0%)。不同科室分离株的耐药率不同,其中以ICU分离株耐药率最高。结论头孢哌酮-舒巴坦和碳青霉烯类药物对鲍曼不动杆菌的体外抗菌活性最强。MDRAB已经达到47.7%,并发现有2.8%菌株为泛耐药株。在不同医院、不同科室的鲍曼不动杆菌对抗菌药物的敏感性有很大差异。Objective To investigate the antimicrobial resistance of A. baumanii isolated from 12 hospitals in different cities all over China in 2007. Methods A total of 2 718 nonduplicate strains of A. baumannii were collected. Antimicrobial susceptibility was determined by disk diffusion method in every center. The antimicrobial susceptibility data was analyzed by WHONET 5.4 software. Results Cefoperazone-sulbactam is the most effective agent (72. 1%) against A. baumanii among 14 antimicrobial agents. The susceptibility rates to imipenem and meropenem were 60.6% and 55.5%, respectively, fol- lowed by minocycline (50.8%) and amikacin (42.9%). The antimicrobial activity of some antimicrobiat agents was significantly different among hospitals. The data collected from 12 hospitals demonstrated that the prevalence of multidrugresistant A. baumannii (MDRAB) was up to 47. 7% (4.7%-64. 0%). The resistance of A. baumannii varied from one department to another. The highest was in ICU. Conclusions Cefoperazone-sulbactam and carbapenems are the most effective antimicrobial agents against A. baumannii in vitro. However, the prevalence of MDRAB and PDRAB is up to 47.7 % and 2.8 %, respectively. The susceptibility of A. baumannii to certain antimicrobial agents varies significantly from one hospital or department to another.
