摘 要:目的研究本院临床分离铜绿假单胞菌产金属β内酰胺酶的基因型、耐药性及同源性。方法用2-巯基丙酸或EDTA协同试验筛选临床分离的亚胺培南耐药的铜绿假单胞菌。协同试验阳性菌株用PCR法检测金属酶基因型,脉冲场凝胶电泳(PFGE)分析产金属酶菌株同源性。结果31株亚胺培南耐药铜绿假单胞菌协同试验阳性5株,PCR扩增有4株阳性,为金属酶IMP-9型,均分离自神经科重症监护病房,PFGE结果显示为同一克隆株。产酶株对阿米卡星敏感或中介,对其他临床常用抗生素均高度耐药。结论本院神经科重症监护病房有产IMP-9型金属酶铜绿假单胞菌流行,应注意进行临床检测和监控。Objective To investigate the genotypes, antibiotic susceptibility and homology of metallo-β -lactamase-producing P. aeruginosa isolated from our hospital. Methods 2-mercaptopropanic acid disc or EDTA synergy test was used to screen metallo-β-1actamase producing and imipenem-resistant P. aeruginosa. PCR was used to identify metallo-β-lactamase genotypes. The homology of these strains was analyzed by pulsed-field gel electrophoresis (PFGE). Results A total of 31 P. aeruginosa isolates were identified, 5 of which produced metallo-β-lactamase as tested by 2-mercaptopropanic acid-disc or EDTA synergy test. Four strains producing IMP-9 were confirmed by PCR and sequencing. PFGE revealed these 4 strains isolated from Department of Neurology belonged to the same clone. Conclusions IMP-9 metallo-β-lactamase producing P. aeruginosa strain is prevalent in our hospital. The emergence of metallo-β-lactamase producing P. aeruginosa will be a serious problem.
