摘 要:目的研究葛根素对子痫前期(PE)患者相关Bax、bcl-2及Caspase-3在胎盘绒毛中的表达的影响,探讨葛根素治疗PE对凋亡相关表达的影响及治疗机制。方法选择正常妊娠妇女60例,硫酸镁治疗后PE患者60例(对照组),硫酸镁与葛根素治疗后PE患者54例(研究组),分娩后取胎盘组织,采用免疫组织化学方法检测凋亡相关蛋白的BCL-2,BAX,Caspase-3表达情况;采用末端脱氧核苷酸转移酶介导脱氧核苷酸切口末端标记法(TUNEL法)检测胎盘细胞凋亡情况。结果研究组胎盘绒毛滋养细胞的凋亡率明显高于正常妊娠组,但低于对照组(P〈0.05)胎盘绒毛中有Bax、bcl-2、Caspase-3表达主要分布于合体滋养层细胞,研究组Bax和Caspase-3阳性表达率分别为(49.6±5.7)%和(29.5±4.1)%,与正常妊娠比较,表达水平显著增加(P〈0.01),与对照组比较,Bax、Caspase-3表达水平显著降低,(P〈0.01)。结论胎盘组织中Bax、Caspase-3在子痫前期发病中可能起重要作用;葛根素治疗能使胎盘绒毛Bax、Caspase-3表达改变,抑制胎盘绒毛的细胞凋亡,提高子宫-胎盘-胎儿血供。Objective To investigate the expression of apoptosis associated Bax, bcl-2 and Caspase-3 in human placenta of preeclampsia (PE), and to evaluate the influence of Puerarin on apoptosis associated gene and the efficiency mechanism. Methods Placental tissues from 60 normal gravidas, 60 post-treatment women with magnesium sulfate and 54 post n with magnesium sulfate and Puerarin were collected. TUNEL was used to detect apoptosis rate and immunohistochemical method was used to detect expressions of Bcl-2, Bax and Caspase-3 in the trophocytes of placenta.Results Under the detection of TUNEL technique, it was found that there were more numbers of apoptotic trophoblasts in the placenta of post-treatment with magnesium sulfate and Puerarin than normal gravidas, and fewer apoptotic cells appearing than post-treatment with magnesium sulfate .The expressions of Bax, bcl-2 and Caspase-3 were evident in human placental.dicating positive staining of Bax, and Caspase-3 were respectively49. 6± 5.7 and 29.5 ± 4.1 in post-treatment with magnesium sulfate and Puerarin group, which were significantly higher than there of the normalgravidas.and were significantly lower than there of the post-treatment with magnesium sulfate groups. Conclusion Bax, and Caspase-3 in placenta may play an important role in the pathogenesis of PE, the Puerarin could reduce the expression of BAX and Caspase-3 of placental, and increase the blood supply of uterus-placenta-fetus and improve therapeutic efficiency.
