探讨SKY椎体成型术的常见并发症及其防治

摘　　要：目的:探讨SKY椎体成型术的常见并发症及其主要发生机制和防治措施。方法:对过去5年间在我院行SKY椎体成型术所发生的并发症进行回顾性分析。结果:SKY椎体成型术治疗67例73个椎体,其中61例67个椎体在术中及术后均未见并发症的发生,3例老年性骨质疏松症所致的压缩性骨折中3个椎体在术中发生少量的PMMA(聚甲基丙烯酸甲脂)渗漏,1例1个椎体在术中发生L4腰动脉损伤,2例2个椎体在术后发生病变的相邻椎体继发性压缩性骨折。发生并发症的比率,按病例计算为8.96%(6/67),按治疗的椎体数计算为8.22%(6/73)。对这6例患者术后进行必要的对症治疗并作6～12个月的随访,其症状基本解除。结论:PMMA的渗漏是SKY椎体成型术中最常见的并发症之一,脊髓圆锥以下的少量渗漏是可以不产生特别症状的。Objective To discuss the common complications, prevention, and management in SKY percutaneous kyphoplasty. Methods Data of 67 patients who had undergone SKY percutaneous kyphoplasty from Jan 2003 to Jan 2008 were retrospectively analyzed. Results In all 67 patients （73 vertebras）, there were no intraoperative or postoperative complications occurred in 61 patients （67 vertebras）, leakage of polymethylmethacrylate （PMMA） occurred during operation in 3 cases （3 vertebras） with osteoporotic compressive fracture, L4 lumbar arteries injury occurred during operation in 1 case （1 vertebra）, secondary compressive fracture occurred after operation in 2 cases （2 vertebraes）. The complication rate was 8.96% （6/67） according to the number of patients, or 8.22% （6/73） according to the number of vertebras. In 6 patients who had complications and received symptomatic management and follow-up （6 - 12 months）, the clinical symptoms were basically disappeared. Conclusion PMMA leakage is one of the most common complications in SKY percutaneous kyphoplasty. Modicum PMNA leakage below the spinal conus wouldn＇t cause any special symptoms.
