青年急性冠脉综合征大动脉扩张性、内皮功能相关性研究

摘　　要：目的探讨大动脉扩张性、内皮功能与青年急性冠脉综合征患者发生冠脉事件的相关性研究。方法采用Complior动脉脉搏波传导速度测定仪和ALOKA5500高频超声检测仪测量青年急性冠脉综合征组（n=44）、青年非急性冠脉综合征组（n=49）以及健康对照组（n=50）大动脉扩张性、内皮功能（FMD）、非内皮依赖性血管扩张（NGT-MD）。结果青年急性冠脉综合征组大动脉扩张性降低和内皮功能异常明显低于其他两组（P〈0．05），大动脉扩张性和内皮功能与冠脉事件呈正相关（分别为r=0．359，P〈0．05；r=0．427，P〈0．05）。结论大动脉扩张性和内皮功能是急性冠脉事件的有效预测指标之一，大动脉扩张性降低和内皮功能异常可能加剧动脉粥样硬化斑块的不稳定，与斑块破裂有关。Objective To explore the relationship between distensibility of large arteries, endothelial function and coronary event in young patients with acute coronary syndrome. Methods According to clinic data, there were divided into 3 groups：young patients with ACS group （n = 44）, young patients with no ACS group （ n = 49 ） and health control group （ n = 50）. Automatic pulse wave velocity measuring system （Complior） was applied to examine carotid-femoral PWV as an index reflecting distensibility of large arteies. Ultrasound （ALOKA5500） was applied to examine the flow-mediated vasodilataion of the brachial artery as an index reflecting endothelial function and the widening of internal diameter of brachial artery using nitroglycerin as index reflecting non-endothelial-dependent blood vessel dilation. Results Distensibility of large arteries and endothelial function in group of young patients with acute coronary syndrome was significantly lower than that of other groups （P 〈 0. 05 ）. A significant positive correlation between distensibility of large arteries, endothelial function and coronary event was found in all of the subjects（ respectively, r = 0. 359, P 〈 0. 05 ; r = 0. 427, P 〈 0.05 ）. Conclusion Distensibility of large arteries and endothelial function are indicative indexes for acute coronary syndrome, they seriously promote artherosclerosis, and might perhaps cause atherosclerosis plaque destory.
