摘 要:目的:探讨18F-FDGPET/CT在诊断肺部占位性病变中的价值。方法:对100例肺部占位性病变进行回顾性分析,比较PET/CT与PET及CT的诊断效度、不同大小肿瘤的SUV、不同病理类型肺癌的SUV。结果:①PET/CT的灵敏度、阴性预测值、准确率优于CT(P<0.01),PET/CT的特异度优于PET(P<0.05)。②依据肿瘤大小分为<3cm、3~5cm、>5cm组,应用单向方差分析对比不同组间SUV,3组间SUV均有差异,P<0.01。各组间SUV两两比较,发现小于3cm组SUV<3~5cm组SUV<5cm以上组SUV。③鳞癌组与腺癌组SUV无显著差异(P=0.821,P>0.05)。结论:18F-FDGPET/CT在诊断肺部占位性病变中将PET代谢学特征密切结合CT的形态学表现,可大大提高各项诊断效率,达到优势互补的目的。Purpose: To evaluate the value of ^18F- FDG PET/CT in diagnosing pulmonary occupying lesions. Methods: One hundred cases of pulmonary occupying lesions were retrospectively analyzed. The diagnosis validity of PET, CT, PET/ CT; the SUV of different size occupying lesions; and the SUV of different pathology occupying lesion were compared respectively. Results: ①The sensitivity , predictive value and accurate rate of PET/CT were higher than those of CT(P 〈 0.01), the specificity of PET/ CT was higher than that of PET(P 〈 0.05) . ② According to the size of tumor, cases were divided into 3 groups: 〈 3cm group, 3 - 5cm group and 〉 5cm group. SUV of different groups were compared by one - way analysis of variance. The SUV of 〈 3cm group was lower than that of 3 - 5cm group and 〉 5cm group. The SUV of 3 - 5cm group was lower than that of 〉 5cm group.③ There were no significant difference between the SUV of squamous carcinoma and that of adenocarcinoma(P = 0. 821, P 〉 0.05) . Conclusion: Combined biological metabolism character with morphologic appearance, PET/CT can improved the diagnosis validity consumedly.
