64层螺旋CT门静脉重建技术在门静脉病变中的诊断价值

摘  要:目的探讨64层螺旋CT门静脉造影重建技术在门静脉病变中的诊断价值。方法对60例门静脉系统疾病患者行64层螺旋CT门静脉造影。重建技术有多平面重建(MPR)、最大密度投影(MIP)和容积再现(VR)。结果肝动脉-门静脉瘘5例,门静脉栓塞41例,肝硬化门静脉高压侧支形成8例,门静脉海绵样变1例,邻近病变的门静脉受侵5例。多层螺旋CT血管重建技术MIP及VR能清楚显示门脉主干的病变,门脉的侧支血管和邻近病变的门脉受侵;MPR较直观显示病变整体形态、范围及门静脉管腔变窄程度。结论64层螺旋CT门静脉造影重建技术是显示门静脉病变有效的诊断方法。Objective To evaluate the value of portal vein image reconstruction techniques in diagnosis of portal vein diseases using 64-slice spiral CT. Methods Sixty cases of portal vein diseases were undergone portal vein image reconstruction with 64-slice spiral CT, including multiplanar reconstruction (MPR), maximum intensity projection (MIP) and volume rendering (VR). Results Among 60 cases, hepatic artery portal fistula 5 cases, portal vein embolism 41 cases, portal sponge degeneration 1 cases, 8 cases collateral vessel with liver cirrhosis and portal hypertension, and the portal invasion by adjacent lesion 5 cases. MIP and VR clearly showed lesion in the main portal vein, portal branches vessles and the portal invasion by adjacent lesion; MPR directly showed the whole shape, the extent and lumen narrowing. Conclusion The portal vein image reconstruction technique is an effective method in diagnosis of portal vein diseases.
