摘  要:目的探讨影像学检查在骨髓间质干细胞(mesenchymal stem cells,MSCs)治疗缺血性脑卒中实验研究中的应用价值。方法雄性成年SD大鼠,用随机数字表法分假手术组(Sham组,n=5)、脑缺血组再灌注组(MCAO组,n=25)、PBS治疗组(n=25)和MSCs治疗组(n=25),每组分别于造模成功后1、3、7、14、28 d行CT灌注成像(CT perfusionimaging,CTPI)、MRI检查、神经功能评分、2,3,5-三苯基氯化四氮唑(TTC)染色、HE染色及微血管密度分析。结果Sham组每时间点血流动力学未见明显异常,脑组织未见明显梗死区域。MCAO组、PBS组、MSCs组:①CTPI参数图、T2WI图与TTC染色显示梗死体积差异无统计学意义;MSCs组与MCAO组、PBS组比较,7、14、28 d梗死体积差异有统计学意义(P<0.05)。②MSCs组与MCAO组、PBS组比较,rCBF在7、14、28 d升高,差异有统计学意义(P<0.05);神经功能评分减低明显,微血管密度明显增高,HE染色示缺血周边区细胞变性坏死程度减低。MSCs组3 d及7 d在梗死灶周围见点状长T1、短T2信号,14 d及28 d低信号向梗死病灶方向弥散,强度减低。结论CTPI和MRI影像技术可以从形态学及血流动力学方面观察MSCs治疗缺血性脑卒中的疗效,磁标记MSCs的MRI检查实现了活体追踪标记细胞在脑内的存活、迁移及分化。Objective To investigate the value of imaging application to evaluate the therapeutic effect of mesenchymal stem cells (MSCs) transplantation in the treatment of ischemic stroke. Methods Totally 80 adult male Wistar rats were divided into sham operation group (n = 5 ), isehemie repeffusion (MCAO) group (n = 25), PBS treated group (n = 25 ) and MSCs treated group (n = 25). MSCs were isolated from the bone marrow of femur and tibia of male Wistar rats at the age of 4 to 6 weeks and identified with flow cytometry followed by SPIO labeling. MCSs of 2 × 10^6 were injected into ischemic reperfusion rats through tail vein to MSCs treated group 24 h after reperfusion, and 1 ml PBS were given to PBS group at the same time point. In 1,3,7, 14 and 28 d after the establishment of experimental models, CT perfusion imaging (CTPI) , MR imaging, modi- fied neurological severity scoring, 2-3-5-triphenyltetrazolium (TTC) staining, HE staining and microvessel density analysis were carried out. Results There was no infarction and hemodynamic was normal at each time point in Sham group. In MCAO group, PBS group, and MSCs group, there was no significant difference in the volume of infarction among CBF and CBV values of CTPI, T2WI images and TTC staining. The infarction volume of MSCs group in 7, 14 and 28 d was significantly smaller than MACO and PBS groups ( P 〈0.05 ). MSCs group had significantly increased rCBF compared with MCAO and PBS group in 7, 14 and 28 d (P 〈 0.05 ). But neurological score was reduced, mierovessel density raised, and HE staining showed cell degeneration and reduced necrosis. Long-T1, short-T2 signal was seen around the infarction in 3 and 7 d, and the low signal diffused to the direction of the infarction and the intensity reduced in MSCs group. Conclusion CTPI and MRI can be used to observe the effects of MSCs in the treatment of ischemic stroke about morphology and homodynamics, track the survival, migration and differentiation of magnetic labeled cells in living
