两种不同方法治疗不稳定型粗隆间骨折的早期疗效比较

摘　　要：目的比较双极股骨头置换术(BHR)、动力髋螺钉(DHS)治疗不稳定型粗隆间骨折的早期疗效。方法采用回顾性分析方法,分析我院在2003年1月～2008年12月31日采用BHR、DHS两种不同方法治疗57例不稳定型股骨粗隆间骨折病人的临床资料及治疗结果,从平均住院时间、平均手术时间、并发症发生率、术中平均出血量、髋关节功能恢复情况等方面对两种不同治疗方法进行比较评价,并作统计学分析。结果BHR组的平均手术时间为(73±19)min,DHS组平均手术时间为(96±33)min,组间比较有显著性差异(P<0.05)。BHR组1年后改良髋关节Harris评分优良率(87.5%),DHS组1年后改良髋关节Harris评分优良率(68.3%%),组间比较有显著性差异(P<0.05),在并发症发生率方面,BHR组是18.75%,DHS组是19.5%,两组无显著性差异(P>0.05),在平均住院时间方面,BHR组平均(34±26)d,DHS组平均(30±20)d;在术中平均出血量方面,BHR组平均(200±151)ml,DHS组平均(200±125)ml,组间均没有显著性差异(P>0.05)。结论BHR操作时间短、术中出血量少、并发症少、疗效好,是治疗不稳定型粗隆间骨折的理想方法之一,尤其适应于骨质疏松性老年不稳定型粗隆间骨折。Objective To compare the early curative effects of bipolar hemiarthroplasty（BHR） and dynamic hip screw（DHS） on femoral unstable intertrochanteric fracture and to offer basis on choosing the proper treatment method in clinic.Methods The clinical and radiological records of 57 patients with femoral unintertrochanteric fracture treated by BHR（l6 cases） and DHS（41 cases） from January 2003 to December 2008 were retrospectively reviewed.All these patients were followed-up for 6 months to 1 year.The average days,operation time,incidence of complications,mean blood loss in operation and the functional evaluation of hip joint were compared and analyzed.Results There was obvious difference between BHR（（73±19） min） and DHS groups（（96 ±33） min）（P0.05）.The rate of excellent and good in BHR group was 87.5%,which was higher than that of DHS group （68.3%）（P0.05）.There were no difference between two groups in the incidence of complications（BHR group 18.75%,DHS group 19.5%）,the mean hospitalization period（BHR group（34±26） d,DHS group（30±20） d） and the mean blood loss in operation（BHR group（200±151） ml,DHS group（200 ±125） ml）（all P0.05）.Conclusions The BHR with advantages of little complication,less bleeding,shorter operation time,and better effect,is one of the best alternative treatments to the aged patients with osteoporosic unstable intertrochanteric fracture.
