小肠出血的气钡双对比造影诊断

摘　　要：目的:评价气钡双对比造影对小肠出血的诊断方法。方法:对便血或大便潜血患者,临床排除胃及结肠出血后,再行小肠口服法气钡双对比重造影检查,对阳性发现的49例诊断结果与病理结果进行对照分析。结果:术前明确诊断44例(89.8%),符合诊断5例(10.2%)。其中平滑肌瘤11例,平滑肌肉瘤18例,表现为腔内结节或较大的腔内外肿块,其表面伴有小溃疡或不规则溃疡。肠癌9例,表现肠管局限充盈缺损或管腔狭窄、僵硬、粘膜破坏。局限肠炎2例,表现肠管窄、粘膜增粗,脂肪瘤2例、息肉2例、憩室3例、血管瘤2例。结论:对于临床便血患者高度怀疑小肠出血者选择气钡双对比造影检查,视为最简便易行、又经济的方法。Objective：To evaluate the air-barium double contrast radiography for the diagnosis of small intestinal bleeding.Methods： Patients with blood in the stool or fecal occult blood, clinical exclusion Of the stomach and colon bleeding, and then the small intestine of oral contrast air-barium double contrast examination by 49 eases found positive results with the pathological diagnosis of the results of com- parative analysis. Results： 44 eases of a clear diagnosis （89.8 % ）, consistent with the diagnosis of five eases （ 10.2% ）. Of which 11 eases of leiomyoma, leiomyosarcoma, 18 eases showed nodular or large cavity inside and outside the tumor cavity, its surface with small ulcers or irregular ulcers. 9 eases of eolorectal eaneer, the performance of intestine or luminal filling defect limited to a narrow, rigid and mueosul damage. The two eases of local enteritis, the narrow lumen, mueosal ttfickening; two eases of lipoma, polyps in two eases, 3 eases of diverticulum, 2 eases of hemangioma. Conclusion： Clinical hematochezia for suspected small intestinal bleeding patients with air-barium double-contrast radiograhy, as the most simple and economical method.
