高龄老年不稳定型股骨粗隆间骨折股骨头置换的疗效观察

摘　　要：目的探讨股骨头置换术治疗高龄老年不稳定型股骨粗隆间骨折的临床疗效。方法32例高龄老年不稳定型股骨粗隆间骨折行人工股骨头置换术(A组),对其临床疗效、术后下床时间、并发症等进行评估,并与高龄老年不稳定型股骨粗隆间骨折DHS内固定患者31例(B组)进行对比。结果A组优良29例(90.6%),B组优良21例(70.0%),差异有显著性(χ2=5.034,P<0.05)。其余2项指标A组也明显优于B组。结论股骨头置换术的疗效明显优于DHS内固定,可作为一种积极而有效的治疗方法,值得推广应用。[ Objective ] To investigate the clinical result of the treatment of unstable intertrochanteric hip fractures in aged patients with hip hemiarthroplasty. [Methods] 63 aged patients with unstable intertrochanteric hip fractures were followed up at random. Hemiarthroplasty （group A, in 32 patients） and DHS internal fixation （group B, in 31 patients） were included. The data in each group were collected for statistical analysis on the following aspects： the excellent and good rate, the time of out of bed exercise after operation and complications. [ Results ] 29 cases were excellent and＇ good （90.6%） in group A, and 21 cases （71.0%） in group B. The time of out of bed exercise after oeration was （15±2.5） days in group A, and （67±2.3） days in group B. The complications occurred in 5 patients in group A, and 17 patients in group B. There was significant difference between group A and group B （P 〈0.05）. [ Conclusions ] The hemiarthroplasty is better than DHS internal fixation in treatment of unstable intertrochanteric hip fractures in aged patients, it can be a good treatment method for deserving application and popularization.
