子痫前期患者合并代谢综合征的发病相关因素初探

摘　　要：目的分析子痫前期患者合并代谢综合征的发病相关因素。方法分析2005年5月～2007年5月在中国医科大学附属盛京医院妇产科分娩的84例子痫前期患者(轻度36例,重度48例)及同期分娩的正常晚期妊娠孕妇90例(对照组)合并代谢综合征情况,并比较两组年龄、孕前体重指数和入院时体重指数、血脂、血糖、OGTT、空腹血浆C-肽、空腹血浆胰岛素水平。结果子痫前期组患者代谢综合征发生率、GDM和糖耐量异常的例数均显著高于对照组(P<0.01),孕前体重指数和入院时体重指数也明显高于对照组(P<0.01,P<0.05)。轻度和重度子痫前期患者两组的代谢综合征的发病率、患者年龄、空腹血浆C-肽、血浆胰岛素水平均无明显统计学意义(P>0.05)。结论高体重指数、GDM、糖耐量异常是子痫前期患者合并代谢综合征的相关因素,其中孕前高体重指数的子痫前期患者更易发生代谢综合征。[Objective] To investigate the correlated factor of metabolic syndrome （MS） in patients with preeclampsia. [Methods] Prospectively analysed the data of patients with preeclampsia （n =84） and normal term pregnant women （control group, n =90）, who delivered in Department of Obstetrics, Shengjing Hospital, China Medi- cal University, from May, 2005 to May, 2007. There were 36 cases of mild preeclampsia and 48 cases of severe. Compared and analyzed the age, pre-gestational body mass index and body mass index on admission, OGTT, serum concentration of C-peptide, insulin. [ Results ] The incidences of MS, GDM and gestational impaired glucose tolerance （GIGT）, and BMI on admission and pre-gestafional BMI in preeclampsia group were higher than those in control group （P 〈0.01）. There was no significant difference of MS incidence between mild and severe preeclampsia （P 〉 0.05）. There were more cases of in mild and severe preeclampsia group than those in control group. There were no significant difference in age, the level of C- peptide and insulin （P 〉0.05）. [ Conclusion ] Higher BMI, GDM and GIGT are risk factors of preeclampsia complicated with MS, and the patients with higher pre-gestational BMI are more likely complicated with MS.
