艾滋病常见实证量化诊断的Logistic回归分析

摘　　要：目的探讨艾滋病4种实证的证候量化诊断。方法以调查问卷方法收集1322例艾滋病患者临床四诊资料,采用二分类Logistic回归分析方法对艾滋病常见实证进行证候量化分析。通过计算OR值对证候进行量化,按OR值大小,揭示出各个症状在证候诊断中的权重。结果4个艾滋病实证均被筛选出具有诊断意义的症状(P<0.05),湿热内蕴证有舌苔黄、舌苔腻、脉沉、腰痛、痞满、恶寒、脉滑、恶心;湿热蕴毒证有舌疮、咽痛、泻下畅利、脱发、消瘦、脉数、脉滑、口苦、苔腻、舌红;痰热蕴肺证有咳痰色黄、咳痰色白、低热、咳痰黏稠、胸闷、舌红;风热蕴肤证有皮肤瘙痒、皮肤粗糙、丘疹、口干不欲饮。结论艾滋病实证的特征是实中夹虚,4种实证证候的量化诊断可为临床辨证提供客观依据。Objective To study the symptom quantitative diagnosis of four excess syndromes of AIDS.Methods The clinical data of 1322 AIDS patients were collected with questionnaire and the commonly seen excess syndromes were analyzed with logistic regression analysis for a symptom quantitative diagnosis.The quantification of symptoms is done according to the OR value to find out the weight of each symptom in diagnosis.Results The meaningful symptoms were found in all the four excess syndromes（P〈0.05）. The syndrome of damp heat retention owns the symptoms of yellow tongue coating, sticky tongue coating, deep pulse, lumbar pain, epigastric fullness, aversion to cold, rolling pulse, and nausea. The syndrome of damp heat retention with toxin owns the symptoms of tongue ulcer, sore throat, diarrhea, hair-falling, emaciation, rapid pulse, rolling pulse, bitter taste in mouth, sticky tongue coating, and red tongue. The symptoms of the syndrome of phlegm heat in the lung are cough with yellow sputum, cough with white sputum, low fever, sticky sputum, fullness in chest, and red tongue. The symptoms of the syndrome of wind heat retaining in skin are itching, rough skin, papule, and thirst but without desire to drink. Conclusion The feature of excess syndromes of AIDS is excess complicated with deficiency. The symptom quantitative diagnosis of the four excess syndromes provides the objective evidence for clinical practice.
