摘 要:背景白细胞介素(interleukin,IL)-18是一种新近发现的促炎症细胞因子,其血清水平与急性胰腺炎(acute pancreatitis,AP)严重程度及合并胰外脏器损伤方面存在相关性,但有关IL-18在重症急性胰腺炎(severe acutepancreatitis,SAP)合并肺损伤中的作用却鲜有报道。目的观察IL-18在SAP大鼠肺组织中的表达,探讨IL-18在SAP合并肺损伤中的作用。方法采用4%牛磺胆酸钠逆行注入胰胆管诱发SAP模型。Sprague-Dawley大鼠32只,随机分为对照组(health control,HC)和SAP6h、12h、18h组。动态监测血清淀粉酶(AMS)、肺湿干重比率,留取标本在光镜下观察胰腺及肺组织病理学改变,采用免疫组化方法检测IL-18在肺脏中的表达和定位,并用Western blotting方法检测肺内IL-18蛋白的表达。结果SAP组各时间点血清淀粉酶、肺湿干重比率均显著高于对照组(P<0.01);SAP各组胰腺和肺组织病理损伤程度随时间明显加重。免疫组化结果显示,SAP组中IL-18在肺泡巨噬细胞胞浆中的表达较HC组明显增多。Western blotting结果显示,SAP组各时间点IL-18表达明显升高,与HC组相比均具有显著性差异(P<0.01)。结论SAP时肺组织中IL-18主要由肺巨噬细胞生成,IL-18可能在SAP合并肺损伤中发挥重要作用。Background Interleukin(IL)-18 is newly discovered proinflammatory cytokine, the serum level of IL-18 is closely associated with the severity and extrapancreatic organ dysfunction in acute pancreatitis(AP), however, few studies have focused on its changes and significance in lung injury in this disease. Aims To observe the expression of IL-18 in rat lung tissue from severe acute pancreatitis (SAP), and to evaluate the role of IL-18 in SAP with lung injury. Methods A rat model of SAP was induced in Sprague-Dawley rat by retrograde infusion of 4% sodium taurocholate into the biliary-pancreatic duct. Thirty-two rats were randomly divided into four groups: health control group(HC), SAP 6 h, SAP 12 h and SAP 18 h groups. The level of serum amylase, wet-dry ratio of lung weight was measured. The expression and distribution of IL-18 in lung tissue were measured by immunohistochemistry and Western blotting. Result The levels of serum amylase, wet-dry ratio of lung weight was significantly increased in the 6h,12h and 18h SAP groups compared with those in control group (P 〈 0.01). The histopathological injuries of pancreatic and lung tissues were gradually aggravating along with the time extension in different SAP groups. The expression of inactive and mature IL- 18 protein in lung detected by Western blotting and immunohistochemistry was significantly increased in pulmonary macrophage in all SAP groups. Conclusions Pulmonary macrophage are the main source of IL-18, IL-18 may play a pivotal role in the pathogenesis of SAP complicated with lung injury.
