对延边朝鲜族急性心肌梗死相关因素的临床分析

摘　　要：[目的]探讨延边朝鲜族急性心肌梗死病人的发病及预后特点。[方法]回顾分析531例延边朝鲜族冠心病急性心肌梗死(AMI)患者,分别记录其一般情况,病史特征,化验检查和心电图,诊断、治疗及预后等,比较不同性别AMI患者的临床特征、不同部位的死亡差异。[结果]与男性相比,女性患者AMI患病率低,年龄偏大,高血压和糖尿病患病率高,病死率高(P﹤0.01);男性患者吸烟、饮酒史多,高脂血症患病率高(P﹤0.01)。广泛前壁AMI发生率最高(24.29%),其次是下壁AMI(23.92%),广泛前壁、下壁的死亡数占总死亡病例的一半以上,广泛前壁合并下壁的病死率最高25%。[结论]延边朝鲜族AMI以前壁和下壁为主,男性仍是发生AMI的主要人群,不同性别A-MI患者入院时临床特征差异显著,女性病死率高。广泛前壁合并下壁心肌梗死的病死率显著高于急性心肌梗死平均病死率。To investigate the incidence and prognosis of AMI suffered by Korean Chinese in Yanbian. [Methods] Reviewed 531 Korean Chinese patients suffered from the AMI, respectively wrote down their general information, characteristics of medical history, chemical examinations, ECG results, diagnostic results, particular treatment and prognosis records and others. Then compared AMI patients＇ clinical characteristics and infarctus difference of various parts. [Results] Comparing with male patients, the female patients had lower incidence of AMI, but higher incidences of hypertension and diabetes, higher rate of infarctus and they were older. The male patients had longer smoking, drinking history, and higher incidence of hyperlipidemia （P﹤0.01）. The incidence of extensive anterior wall AMI was highest （24.29%） and second incidence was inferior wall AMI （23.92%）. The death rate of extensive anterior and inferior wall AMI accounted for more than half of the total AMI death patients, that of extensive anterior AMI combined with inferior AMI was highest （25%）. [Conclusion] AMI Korean Chinese patients most suffer from extensive anterior wall and inferior wall AMI and the males are the main patients of A-MI. There are obvious clinical characteristics among AMI patients of different genders when being in hospital. The females undergo higher rate of infarctus and the infarctus rate of extensive anterior AMI combined with inferior AMI is notably higher than average AMI.
