摘 要:目的探讨布拉酵母菌对小儿支气管炎和肺炎抗生素相关性腹泻(AAD)的预防作用。方法195例患有支气管炎和肺炎住院儿童完全随机分成2组,治疗组95例,自开始使用抗生素起口服布拉酵母菌散剂250mg/次,2次/d;对照组100例,仅使用抗生素,出现腹泻后2组患儿均加用蒙脱石散剂和补液治疗。比较2组患儿的抗生素相关性腹泻率。结果治疗组8例(8.4%)患儿发生AAD,对照组25例(25.0%)患儿发生AAD,2组患儿AAD发生率差异有统计学意义(r=9.52,P〈0.05)。治疗组AAD患儿腹泻持续时间(3.0±1.8)d,较对照组(5.0±2.0)d短,差异有统计学意义(t=7.4,P〈0.05)。治疗组中,≤3岁儿童AAD发生率比〉3岁儿童高,差异无统计学意义(P〉0.05)。对照组中,≤3岁儿童AAD发生率比〉3岁儿童明显升高,差异有统计学意义(P〈0.05)。结论布拉酵母菌能有效预防小儿抗生素相关性腹泻。Objective To assess the preventive efficacy of Saccharomyces boulardii for children with bronchitis and pneumonia on antibiotic-associated diarrhea. Methods One hundred and ninety-five hospitalized children diagnosed with bronchitis or pneumonia were included in a randomized parallel placebo-controlled trial. Children received standard antibiotics and S. boulardii for 7-10 days. The placebo group received standard antibiotics only. The incidence of antibiotic-associated diarrhea was compared in two groups. Results Of the 195 eligible patients, children receiving Saccharomyces boulardii developed significantly less AAD ( with incidence of 8.4% ) compared with 25.0% in placebo group ( P 〈 0.05 ). The diarrhea duration in treatment group was significantly shorter than that in placebo group ( P 〈 0.05 ). Conclusion The prophylactic use of Saccharomyces boulardii together with antibiotics resulted in a significant reduction of AAD with no serious adverse reactions and shortened duration of diarrhea.
