摘 要:目的探讨杨浦区公务员体检中的14C尿素呼气试验结果以揭示幽门螺旋杆菌感染在这一群体中的分布情况。方法收集2008年9月份在上海市杨浦区中心医院体检中心进行14C尿素呼气试验检查的杨浦区各机关公务员的一般资料,并采用现场发放问卷的方式收集其生活习惯等相关信息。利用幽门螺旋杆菌测试仪(HUBT-01型,深圳中核海得威生物科技有限公司)对呼气标本进行检测,结果≥100dpm为幽门螺旋杆菌阳性。将呼气试验检查结果与受检查者的一般资料进行相关性分析。结果共有1219名公务员接受呼气试验检查,其中男性721名,女性498名,平均年龄50.7±15.1(22~90)岁。幽门螺旋杆菌感染的总体阳性率为45.4%,男性的感染率为44.9%,女性为46.2%,两种性别间无显著差异(P=0.682)。平均检查值为(314.2±475.4)dpm(0~3205dpm,中位数为70dpm),男性为(262.1±377.0)dpm,女性为(389.6±581.7)dpm,性别间有显著差异(P=0.000)。将受检查者分为男、女各6个年龄组,男性的检查值高峰出现在21~30岁组[平均值为(368.3±450.2)dpm],女性的检查值高峰出现在41~50岁组[平均值为(489.2±618.3)dpm]。不同工作单位间的检查结果差别较大,感染率最高的为87.5%,最低的为9.1%。按照感染率高低将各单位分为3组并与受检查者的一般资料及调查问卷信息进行相关性分析,结果表明:感染率较高的单位受检查者年龄较低、在职人员比重较大、饮食较不规律、工作强度较高以及吸烟、饮酒者所占比例较高。结论公务员群体中的幽门螺旋杆菌感染率在两种性别间无明显差异,但女性的感染程度显著高于男性。男性及女性的呼气试验检查值均呈单峰分布,男性的检查值高峰在21~30岁时出现,其后随年龄增长而逐渐下降,而女性的高峰则在41~50岁时出现。不同工作单位间幽门螺旋杆菌感染率及感染程度差别较大,总体年龄较低、在职人员�Objective To explore the profile of helicobacter pylori (HP) infection in civil servants by analyzing the outcomes of 14C urea breath test in health examination. Methods The general data of civil servants in government organizations in Yangpu district of Shanghai were collected when they took 14C urea breath test at the health examination center in our hospital in Sep. 2008, along with their living habits information obtained by questionnaire. The HUBT-01 tester for helicobacter pylori detection (Zhonghe Headway Bio-Sci & Tech Co., Ltd, Shenzhen, China) was employed to assay the 14C labeled Urea breath samples and test results no less than 100 dpm were defined as positive for HP infection. Potential correlation was discussed for between test results and general data of the examinees. Results 1219 civil servants took the 14C urea breath test, among which 721 were male and 498 were female, with a mean age of 50.7±15.1(22-90). The overall positive infection rate of helicobacter pylori was 45.4%, and there was no significant gender difference (44.9% for male and 46.2% for female respectively, P=0.682). The maximal and minimal test values were 3205dpm and 0dpm respectively (median=70dpm) while the mean value was (314.2±475.4)dpm, and there was a significant gender difference (262.1±377.0dpm for male and 389.6±581.7dpm for female, P=0.000). The examinees were divided into different age groups (6 for each gender). The peak value for male was observed in the 21-30 age group (mean value 368.3±450.2dpm) meanwhile for female that was in the 41-50 age group(mean value 489.2±618.3dpm). Test results varied between organizations, with the highest and lowest positive infection rate being 87.5% and 9.1%, respectively. Organizations were divided into 3 groups, the positive rates of which being above 50%, between 50% and 33.3%, and below 33.3% respectively. Correlation analysis indicated that groups with higher positive rate tend to have staff that are younger, have irregular dietary
