摘 要:目的:探讨生长激素(GH)、胰岛素样生长因子-Ⅰ(IGF-Ⅰ)、胰岛素样生长因子结合蛋白-3(IGFBP-3)与特发性胎儿生长受限(FGR)的关系。方法:采用巢式病例对照的研究设计方法,使用放射免疫分析法,检测42例FGR患者(特发性FGR组)和42例适于胎龄儿(正常对照组)脐血清中GH、IGF-Ⅰ、IGFBP-3水平,通过配对设计差值t检验、Pearson相关分析对实验结果进行统计分析。结果:①特发性FGR组GH水平高于正常对照组(P<0.01);特发性FGR组脐血IGF-Ⅰ、IGFBP-3水平均明显低于正常对照组(P<0.01);特发性FGR组脐血IGF-Ⅰ/IGFBP-3的比值低于正常对照组(P<0.01);②脐血GH水平与出生体重负相关(r=-0.443,P=0.000),脐血IGF-Ⅰ、IGFBP-3水平与出生体重正相关(r=0.497,P=0.000;r=0.331,P=0.000);脐血GH水平与IGF-Ⅰ水平负相关(r=-0.290,P=0.000),脐血IGF-Ⅰ水平与IGFBP-3水平正相关(r=0.245,P=0.024)。结论:GH、IGF-Ⅰ及IGFBP-3可能参与胎儿生长发育过程,是特发性胎儿生长受限发生的重要影响因素。Objective:To investigate the relations of growth hormone (GH), insulin-likegrowth factor Ⅰ(IGF-Ⅰ), and insulin-like growth factor binding protein( IGFBP-3)with idiopathic fetal growth restriction. Methods: Blood samples were collected from umbilical cord in forty-two cases of fetal growth restriction infant with unclear etiologies ( FGR group) and forty-two cases of appropriate for gestational age infant (control group)by nested case-control study.The concentrations of GH, IGF- Ⅰ, and IGFBP-3 were measured by radioimmunoassay, and the results were analyzed and compared by paired samples t-test and correlation analysis.Results:The serum GH of umbilical cord in FGR group was significantly higher than control group ( P〈 0.0]) ; however, both serum IGF-Ⅰ and IGFBP-3 of umbilical cord were found in FGR group lower than in control group, same as the ratio of IGF-Ⅰ to IGFBP-3 ( P 〈 0.01). The serum GH of umbilical cord was significantly negative correlation with birth weight ( r= - 0.443, P= 0.000) ; but IGF-Ⅰ and IGFBP-3 in umbilical cord was positively related with birth weight( r= 0.497, P= 0.000; r= 0.33), P= 0.000). The serum GH of umbilical cord was significantly negative correlation with IGF-Ⅰ ( r= - 0.290, P= 0.000), the serum IGFBP-3 of umbilical cord was significantly positive correlation with IGF- Ⅰ( r= 0.24.5, P= 0.024).Conclusions:GH, IGF-Ⅰ, and IGFBP-3 are involved in fetal intrauterine growth and play important role in the mechanism of idiopathic fetal growth restriction.
