无高血压的糖耐量减低患者24h动态血压分析

摘　　要：目的观察未合并高血压的糖耐量减低患者24 h血压改变情况。方法回顾分析2005～2007年入院查体患者162例,根据空腹血糖、口服葡萄糖耐量试验(OGTT)或餐后2 h血糖分为健康对照组40例,无高血压的糖耐量减低组37例,无高血压的糖尿病组45例,无糖耐量减低的高血压组40例,记录各组的血脂(TG、TC、HDL、LDL)、偶测血压数值,采用动态血压监测仪记录24 h动脉血压并测量24 h平均收缩压(24 h SBP)、24 h平均舒张压(24 h DBP)及24 h动态脉压(24 h PP)。结果(1)糖耐量减低组与糖尿病组的TG高于健康对照组和高血压组,差异有统计学意义(P<0.05)。(2)糖耐量减低组24 h SBP及24 hDBP与健康对照组差异有统计学意义(P<0.01),而与高血压组差异无统计学意义(P>0.05)。糖耐量减低组24 h PP与高血压组差异无统计学意义(P>0.05)。结论糖耐量减低患者在临床明确诊断高血压前已发生血压调节的明显改变。Objective To observe the alternation of mean artery blood pressure in patients with impaired glucose tolerance without hypertension. Methods One hundred and sixty-two patients were divided into four groups, healthy group （40 cases） , impaired glucose tolerance without hypertension group （37 cases）, diabetes mellitus without hypertension group （45 cases） , hypertension group without impaired glucose tolerance （40 cases） ,according to fasting blood glucose, OGIT or postprandial blood glucose. The TC,TG, HDL, LDL, blood pressure were recorded, and the 24 h SBP, DBP and PP were acquired with ambulatory blood pressure monitor. Results （ 1 ） The triglycerides of impaired glucose tolerance group and diabetes group were higher than healthy and hypertension groups, and the difference was statistically significant（ P 〈 0.05 ）. （2） The 24 h SBP and DBP of patients with impaired glucose tolerance displayed statistical differences（ P 〈 0.01 ）with healthy persons, while no difference with hypertension group （ P 〉 0.05）. Additionally, the 24 h PP showed no significance between impaired glucose tolerance group and hypertension group（ P 〉 0.05）. Conclusion It suggested that the blood pressure of impaired glucose tolerance patients had significant alternation before hypertension diagnosed.
