先天性心脏病介入治疗的并发症分析及其防治

摘　　要：目的:探讨与先天性心脏病(先心病)介入治疗相关的并发症的发生率、发生原因及防治方法。方法:应用心电图和经胸彩色超声心动图的方法观察本院1546例行心脏介入治疗的先心病患者术中和术后并发症,并进行随访。结果:在3～42(平均20.6±13.9)个月的随访期内,1546例先心病患者,介入治疗总成功率97.61%(1509/1546),总并发症发生率9.70%(150/1546),严重并发症发生率1.10%(17/1546),其中Ⅲ度房室传导阻滞5例,重度三尖瓣关闭不全2例,封堵器脱落5例,蛛网膜下腔出血1例,基底节区脑梗死1例,股动脉血栓3例,经相应治疗均治愈,无一例死亡。结论:先心病介入治疗安全、有效,并发症发生率低。术前正确诊断、术中准确操作和术后认真随访可减少并发症的发生。Objective：To investigate the incidence and cause of complications associated with interventional therapy for patients with congenital heart disease（CHD）. Methods：A total of 1546 CHD patients received the interventional therapy procedures in our hospital were retrospectively studied for their complications. All patients were followed up from 3-42 （mean 20. 6 ± 13.9 ） months by electrocardiogram and echocardiography examinations. Results：By follow up study, the technical success rate was 97. 6% （ 1509/1546）, and the incidence of complication was 9. 71% （150/1546） in all patients with CHD. While the serious complication was 1.10% （17/1546） that including 5 cases of m o atrioventricular block,2 of severe tricuspid regurgitation,5 of device shedding, 1 of subarachnoid hemorrhage, 1 of cerebral in- farction in basal ganglia area and 3 cases of femoral artery thrombosis. No deaths occurred during the follow up period of time. Conclusion：Interventional therapy was the safe and effective treatment for CHD and the incidence of complication was low. The complication could be reduced by right diagnosis, excellent hand skill and careful post operative follow up.
