带血管蒂跖骨瓣逆行移位术的应用解剖

摘　　要：目的为应用带血管蒂跖骨瓣逆行移位修复第二跖骨头缺血性坏死提供解剖学依据。方法在30侧经动脉灌注红色乳胶的成人下肢标本和4只足部铸型标本上观测第一、二跖背动脉的起源、走行、分布和吻合情况。结果第一、二跖骨背动脉均有骨膜支供养第一、二跖骨。该两动脉远端分别在第一、二趾蹼缘近侧1.59±0.23cm和1.64±0.20cm处发出趾背动脉并向跖底发吻合支与第一、二跖底动脉构成恒定吻合。结论应用第一或第二跖背血管为蒂的第一、二跖骨近段骨瓣逆行移位可修复第二跖骨头缺血性坏死。Objective To provide an anatomic basis of transposition of reverse vascularized metatarsal bone flap for repairing ischemic necrosis of the second metatarsal bone head. Methods The origin, course, distribution and anastomosis of the first and second dorsal metatarsal arteries were observed on adult lower limb cadaver specimens (30) perfused with red latex and foot cast form specimens (4). Results Both the first and second dorsal metatarsal arteries had periosteal branches for supplying the first and second metatarsal bone. Their distal parts gave off dorsal digital arteries of foot on proximal 1.59±0.23 cm and 1.64±0.20 cm to the first and second web-booted margin. Meanwhile, they gave off anastomostic branches of plantar metatarsal, which anastomosed the first and second plantar metatarsal arteries constantly. Conclusion Transposition of proximal metatarsal bone flap pedicles with the first or second dorsal metatarsal arteries was applied for repairing ischemic necrosis of the second metatarsal bone head.
