新生儿缺血缺氧性脑病血清肌酸激酶及其同工酶、肾功能和血糖动态改变的临床研究

摘  要:目的探讨新生儿缺血缺氧性脑病(HIE)时血清肌酸激酶(CK)和心型同工酶(CK-MB),脑型同工酶(CK-BB),肾功能(BUN,Cr)及血糖动态变化的意义。方法对2005年1月至2007年10月的66例HIE患儿和50例正常新生儿分别在24h内,第7天采集股静脉血,测血清血中总CK,CK-MB,CK-BB,BUN及Cr,血糖采用末梢血。结果(1)HIE组血清CK,CK-MB,CK-BB,BUN,Cr水平高于对照组(P<0.05),均随病情加重而升高,生后24h均达到峰值。(2)中,重度组血清CK,CK-MB,CK-BB,BUN,Cr较对照组比较差异显著(P<0.01)。(3)入院即刻,12h,24h测血糖,重度HIE组与轻、中度HIE在血糖异常种类上有明显差异(P<0.01)。第7日查血糖各组无统计学意义。结论(1)HIE血清中CK,CK-MB,CK-BB,BUN及Cr水平与HIE程度有密切相关性。(2)血清BUN及Cr则是肾损伤的可靠指标。(3)轻,中度HIE以低血糖为主,重度HIE以高血糖为主。Objective: To approach the significance of kinesis change of serum creatine kinase (CK) , MB isoenzyme of creatine kinase ( CK - MB) , ereatine kinaes BB isozyme ( CK - BB) , renal function ( BUN, Cr) and blood glucose in hypoxic - ischemie encephalopathy of newborn (HIE). Methods : To measure the total level of CK, CK - MB, CK - BB, BUN and Cr by collecting femoral vein blood in 24 hours and the seventh day after birth respectively from 66 cases with HIE and 50 normal newborns ( January 2005 to October 2007 ) , the level of blood glucose were measured by using peripheral blood. Results : ( 1 ) The level of CK, CK - MB, CK - BB, BUN and Cr in group HIE were all higher than control group ( P 〈 0. 05 ), increased with the aggravation of patient's conditions and achieved peak value in 24 hours after birth. (2) The level of CK, CK - MB, CK - BB, BUN and Cr were significandy higher in midrange and severe groups than control group ( P 〈0. 01 ). (3) To measure the level of blood glucose in the time when admitted , 12 hours and 24 hours after admitted respectively , the kinds of pathoglyeemia were significantly different in severe group and low - grade and midrange groups. The blood glucose levels were measured in the seventh day with no statistical significance in every group. Conclusions: ( 1 ) The level of CK, CK - MB, CK - BB, BUN and Cr were closely correlated with the degree of HIE. (2) BUN and Cr were reliable indicator of renal injury. (3) Low - grade and midrange groups were main hypoglycemia, severe HIE were majority hyperglycemia.
