摘　　要：目的探讨C臂X线机透视下闭合复位经皮克氏针内固定治疗不稳定儿童肱骨髁上骨折的疗效。方法C臂X线机透视下闭合复位经皮克氏针内固定术治疗儿童不稳定肱骨髁上骨折35例。结果35例均获随访,时间7～13个月。术后无缺血性挛缩,3例出现针孔渗出较多,经加强换药后痊愈。无一例出现肘内翻、尺神经损伤。按Flynn标准评价:优20例,良15例,无一般及差病例。结论C臂X线机透视下闭合复位经皮克氏针内固定治疗儿童不稳定肱骨髁上骨折手术创伤小,固定可靠,可以获得良好的肘关节功能和外形。Objective To explore the therapy effect of unstable supracondylar fracture in children with closed reduction and internal fixation with percutaneous pinning by using C-arm image intensifier. Methods Thirty-five children with unstable supracondylar fractures were treated with closed reduction and internal fixation with percutaneous pinning by using C-arm X-ray perspective. Results Thirty-five patients were followed up from seven to thirteen months. The therapy effects were evaluated with appearance and function of the elbow joint. Pin-tract infections were observed in three patients and were completely healed by frequent dressing change. No patient developed ischemic contracture,cubitus varus and cubital nerve damage. The outcome according to Flynn criteria was graded as excellent in 20 pateints and good in 15 patients. No cases were graded as fair and poor. Conclusions Closed reduction and internal fixation with percutaneous pinning by using C-arm X-ray perspective is a safe,effective method with minimal damage and has good curative effect on children with unstable supracondylar fracture.
