地尔硫卓治疗围手术期重症高血压的研究

摘　　要：目的评价盐酸地尔硫卓注射剂在治疗围手术期高血压中的有效性和安全性。方法开放性病例收集研究。共收集124例非心脏外科手术合并有高血压的患者。先以5～10μg/(kg.min)静脉滴注或泵入,每5分钟递增5～10μg/(kg.min),平均点滴速率在30min内渐增至10～20μg/(kg.min)。观察血压和心率的变化。结果治疗前与治疗后不同时间段对收缩压、舒张压以及心率的作用效果分别为:从治疗前的SBP/DBP208.8±19.4/118.4±10.2(mmHg)变为治疗后10min的SBP/DBP182.4±16.2/95.1±10.2(mmHg),和用药前相比*P<0.05;治疗后60min:SBP/DBP154.5±16.3/84.2±8.4mmHg,和用药前相比**P<0.001。给药前与给药后各时间段的心率无显着性改变(用药前心率:89.4±18.4次/min,用药后60min 81.1±14.7次/min),P>0.05。结论地尔硫卓注射剂治疗围手术期重症高血压安全有效。Objective It was aimed to evaluate the antihypertensive clinical efficacy and safety of continuous intravenous diltiazem infusion into patients with Peri -- operated hypertensian.Methods Opened clinical trial. 124 cases of peri -- operated hypertension were studied. Intravenous diltiazem was administered fist with infusion of 5-10μg/（kg·min）, or with micro--syringe, then gradually increase 5-10μg/（kg·min）once 5 minutes, until the dosage up to 10-20μg/（kg·min）within 30 minutes. The changes of SBP, DBP and heart rate were observed.Remlts The blood pressure and heart rate were monitored and compared before and after 10, 60minutes of intravenous diltiazem used. The effect on blood pressure was significant, SBP/DBP was gradually dropped from 208.8 ± 19.4/118.4±10.2（mmHg） to 182.4±16.2//95.1±10.2（mmHg） after 10 minutes （P〈0.05） and 154.5± 16.3/84.2 ±8.4 （mmHg）after 60 minutes respectively（P〈0.001）. At the same time HE and ECG didn＇t change obvlously（89.4 ±18.4 bpm vs 81.1 ±14.7 bpm,P〉0.05）. Conclusion The data indicate that Intravenous diltiazem injection therapy is effective and safe for patients with peri-operated hypertension.
