摘  要:目的比较自然周期与人工周期方案冻融胚胎移植(frozen-thawed embryo transfer,F-ET)的结局。方法回顾性分析2005年1月至2007年12月在广州医学院第三附属医院进行的986个F-ET周期,将在自然周期方案下进行的F-ET作为自然周期组,共671个周期;在人工周期方案下进行的F-ET作为人工周期组,共315个周期,比较两组间临床妊娠率、种植率、生化妊娠率、流产率及分娩率等指标。同时评估子宫内膜厚度与F-ET结局的关系。结果临床妊娠率、种植率、生化妊娠率、流产率及分娩率分别为:自然周期组22.1%、11.7%、2.1%、18.2%、79.7%;人工周期组23.5%、11.9%、2.9%、18.9%、77.0%。两组间临床妊娠率、种植率、生化妊娠率、流产率及分娩率比较均无统计学意义(P>0.05)。自然周期组较人工周期组的子宫内膜厚度[(9.4±1.4)mm对(8.6±1.2)mm]更理想,差异有统计学意义(P<0.01)。子宫内膜厚度>10mm者临床妊娠率(26.8%对18.9%)和种植率(15.1%对10.6%)均较子宫内膜厚度≤8mm者显著升高(P<0.05)。结论自然周期和人工周期方案在F-ET中的结局相似;自然周期方案较人工周期方案子宫内膜厚度更为理想。子宫内膜厚度>10mm者能获得更高的临床妊娠率及种植率。Objective To compare the outcome of F-ET between in natural cycles and artificial cycles. Methods A retrospective study was conducted on 986 cycles of F-ET between January 2005 and December 2007 in the third Affiliated Hospital of Guangzhou Medical College, natural cycles were 671 and artificial cycles were 315. Compare the clinical pregnancy rate, implantation rate, biochemical pregnancy rate, abortion rate and delivery rate between the two groups. At the same time we also assess the relationship between endometrial thickness and outcome of F-ET. Results The clinical pregnancy rate, implantation rate, biochemical pregnancy rate, abortion rate and delivery rate were 22. 1%, 11.7% 2. 1% ,18. 2% and 79. 7% in the group of F-ET in natural cycles respectively, and were 23.5%, 11.9%, 2. 9%, 18. 9% and 77.0% in the group of F-ET in artificial cycles. There were no significant differences between the two groups with regard to the clinical pregnancy rate, implantation rate, biochemical pregnancy rate, abortion rate and delivery rate ( P 〉 0. 05 ). But mean endometrial thickness prior to transfer was greater in natural cycles [(9.4±1.4) mm vs. (8.6±1.2) mm] ( P 〈 0. 01 ). Significant higher clinical pregnancy ( 26. 8% vs. 18.9% ) and implantation rates ( 15. 1% vs. 10. 6% ) were achieved in cycles where endometrial thickness was 〉 10mm, compared with those in which endometrial thickness was ≤8mm(P 〈 0. 05). Conclusion These findings suggest that both F-ET protocols are equally effective in terms of implantation rate and pregnancy outcome, and greater endometrial thickness was achieved in natural cycles. Besides an endometrial thickness of 〉 10mm prior to transfer is associated with higher clinical pregnancy and implantation rates.
