摘 要:目的探讨改良FWM心脏保存液对离体鼠心低温保存效果。方法20只雄性Wistar大鼠随机分为4组:对照组、改良删液组、HTK液组和K-H组。除对照组外,其余各组取大鼠心脏后,立即分别用各自4℃保存液灌注至心脏停搏,并置于相应的4℃心肌保护液中保存8h,使用Langendorff模型测定心脏功能、检测冠脉流出液的心肌酶、观察心肌的形态学改变以评价各组心肌保护效果。结果与对照组比较,各实验组心功能指标均有明显下降。HTK组LVDP、dp/dtmax、dp/dtmin,均低于改良FWM组(P〈0.01)。而HTK组CF的高于改良FWM组(P〈0.05)和K-H组(P〈0.01)。HTK组GOT、LDH、CK、CTNI、CK-MB、GPT漏出量也高于改良FWM组(P〈0.01)。电镜显示改良FWM组形态学无明显改变仅偶见内质网扩张;而HTK组心肌肌纤维疏松、线粒体轻度水肿、内质网轻度扩张、间质轻度水肿。结论离体鼠心低温保存8h后,心肌功能均有很大程度下降。改良FWM液对离体鼠心的低温保存效果略优于HTK液。Objective Optimal heart-preservation solution is an important factor for the survival of patients who is undergoing heart transplantation. A potassium channel opener nicorandil with heart protective effect was added to the FWM preservation solution, which we studied bofore, for improving the preservation of rat hearts at hypothermic setting in vitro. Methods Male Wistar rats (n = 20) were randomly divided into 4 groups: control, modified FWM, HTK and K-H group. The hearts of rats, except that in the control group, were preserved simply by immersing for 8 horns at 4~C in one of three solutions mentioned above. At the end of the immersion, the hearts were immedistely reperfused with K-H solution ha Langendorff device. Some indices of myocardial function and myocardial enzymes in the coronary effluent were measured . Specimens of the hearts obtained after reperfusion were observed for changes in the ultrastruetures of the myocardittm to assess the protective effect on the heart. Results The heart furtction of rats in three experimental groups decreased significantly with various degree, as compared with that in control group. Mean left ventricular developed pressure (LVDP),dp/dtmax,dp/dtmin in the HTK group were lower significantly than that ha the modified FWM group(P 〈 0.01), while HTK group showed significantly higher coronary flow (CF) than modified FWM group ( P 〈 0.05 ) and K-H group( P 〈 0.01 ). Myocardial enzymes released were decreased remarkably in the modified FWM and HTK groups as compared to those ha the K-H group ( P 〈 0.01), while CTNI、LDH, CK, CK-MB released were increased significantly in the HTK group than those in the modified 17WM group ( P 〈 0.01 ). The findings of the myocardial tdtrastruetures were in favor of modified FWM solution, ha which myofibril of the myocardium was ha good alignment and the endoplasmic reficulmn was distended infrequently.
