摘  要:目的探讨不同体位对婴幼儿先天性心脏病术后肺功能恢复的影响。方法将60例接受体外循环下先心病矫治手术的患儿按照随机数字表法分为试验组与对照组,试验组在术后4h采取半卧位与半侧卧位交错的体位,对照组采用半卧位,监测两组患儿血气等反映肺功能恢复情况的指标。结果试验组在术后8h和24h的动脉二氧化碳分压(PaCO2)均低于对照组,两者相比,P均<0.05,同时术后24h试验组的动脉氧分压(PaO2)要高于对照组,P<0.05;试验组患儿的呼吸机应用时间及ICU住院时间均明显少于对照组,两组相比,P均<0.05。结论先心病患儿术后4h始交替采用半卧位及左、右半侧卧位,可以有效改善患儿血气结果,缩短呼吸机应用时间及ICU住院时间。Objective To examine the influences of different body positions on the pulmonary function in children with con- genital heart disease. Methods Sixty infants after cardiopulmonary bypass were randomly divided into experimental group and control group. The body position was changed for the experiment group in the turn with semi-supine position and semi-lateral position every 4 hours,while semi-supine position only for the control group. Then,the parameters of blood gas analysis were compared between the two groups. Results Compared with the control group,the parameters of blood gas analysis were better,and the time of mechanical ventilation and ICU hospitalization were shorter in the experimental group. Conclusions Taking semi-supine position and semi-lateral position in turn can improve the parameters of blood gas analysis,shorten the time of mechanical ventilation and ICU hospitalization in the infants with congenital heart diseases after cardiopulmonary bypass.
