小儿肾病综合征降脂疗法的临床应用

摘　　要：为了解小儿肾病综合征(NS)综合降脂疗法的临床治疗效果,对49例NS患儿肾病极期、缓解期血浆TC、TG、HDL-C、ApoA_1、ApoB_100的变化进行监测。以24例健康儿童(对照组)血脂作对照。结果:1.NS患儿极期血浆TC、TG、LDL-C、ApoB_100均显著高于对照组(P<0.01),HDL-C,ApoA_1无明显变化。2.进入缓解期,未予降脂治疗的(Ⅰ组)TC、TG、LDL-C水平虽较极期有显著下降,但仍高于正常对照组(P<0.01)。给予降脂治疗的(Ⅱ组)TC、TG、LDL-C、ApoB_100水平与Ⅰ组同期相比显著下降(P<0.05)。提示:肾病缓解期时血脂自然下降程度落后于血浆蛋白和临床症状的改善。综合降脂疗法对NS患儿高脂血症的治疗有明显疗效。To study clinically a comprehensive therapy for correcting lipidemia,the changes in plasma total chol-esterol(TC), triglyceride(TG), high-density lipoprotein-C(HDL-C), low-density lipoprotein-C(LDL-C), apolipoprotein A1 (ApoA1 )and apolipoprotein B100 (ApoB100 )were determined in 49 children with nephrotic syndrome(NS)in the serious and restoration. The plasma lipids in 24 normal children were observed as controls. Results: 1. The TC,TG,LDL-C, ApoB100 in the NS serious stage were obviously higher than in the restoration stage(P<0.01). 2. Being in restoration stage,the TC,TG,LDL-C in the first group(without treatment for correcting lipidemia) were obviously lower than in serious stage, but still higher than in normal subjects. The TC,TG-LDL-C,ApoB100 in the second group(with therapy for correcting lipidemia) were obviously lower than in the first group(P<0.05). Conclusion: In NS restoration stage, the natural correction of lipidemia is slower than the recovery of plasma proteins and clinical symptoms. The comprehensive therapy for correcting lipidemia had a good effect in nephrotic children with lipidemia.
