不稳定型心绞痛患者CD62P和C反应蛋白的测定及临床意义

摘　　要：目的探讨不稳定型心绞痛患者血小板胞浆内α颗粒上的膜糖蛋白（CD62P）含量及血清C反应蛋白（CRP）水平的变化及临床意义。方法应用FACSCalibur流式细胞仪及特定蛋白分析仪测定并比较不稳定型心绞痛患者（UA组）及健康对照者（对照组）的血小板CD62P含量及血清CRP水平，追踪观察6个月后比较UA组不同血小板CD62P含量及血清CRP水平患者急性心血管事件的发生率。结果UA组患者血小板CD62P含量及血清CRP水平均明显高于对照组（均P〈005）；UA组患者血小板CD62P含量与血清CRP水平呈明显正相关（P〈005）：UA组血小板CD62P高含量及血清CRP高水平患者急性心血管事件（心肌梗死、心力衰竭、血管重建术）的发生率（74．28％、71．05％）显著高于CD62P低含量及CRP低水平患者（16.67％、29．63％）（均P〈0．01）结论血小板活化及炎症反应在UA的发生及发展中起着重要作用，其相关指标CD62P及CRP对预测UA患者急性心血管事件的发生及预后具有一定的临床意义。Objective To investigate the platelet activation CD62P and serum C-reactive protein （CRP） levels in patients with unstable angina （UA）. Methods Platelet activation factor CD62P were detected by flow cytometry and CRP were measured by enzyme-immeoassay in 65 UA patients and 67 normal controls. Patients with UA were divided into two groups according to platelet activation CD62P levels： high CD62P group and low CD62P group; or divided into two groups according to serum CRP levels： high CRP group and low CRP group. All patients were follow-up for six months,then comparison the incidence of cardiovascular events,which between high CD62P group and low CD62P group or high CRP group and low CRP group.Results The expression of CD62P and CRP was higher in UA patients than that of NC （ t=2.15 and t=2.17, P〈0.05）. CD62P were positively correlated with CRP in patients with UA （r=0.512, P〈0.05）. The probability of acute coronary events （myocardial infarction, heart failure, blood vessel rebuild） in patients with UA of high CD62P group and high CRP group were higher than low groups （X^2 = 6.72 and X^2 = 6.85, P〈0.01）. Conclusion The platelet activation and CRP may be involved in the development of UA and may be of prognostic value for UA.
